As an Individual and Family Coordinator at Banner Plans & Networks, you will call upon your lived experience navigating in managed care systems. You will be working within workplans, within project plans, and creating policies and procedures to ensure members' choices are heard. You will utilize your strong collaboration skills to liaison with providers, state leaders, state regulators, local community members, and other health plans' representatives inside and outside of Banner Health.

Your work location will be a remote hybrid setting with a home-based office and an in-person territory. Driving within this territory can be as great as two hours each way.

Please note: Personal experience receiving Medicaid services and/or navigating a public health or social service delivery system or experience assisting a family member in navigating a publicly funded system is required for this role.

POSITION SUMMARY

This position will work across and collaborate with BUHP's network provider partners and community service agencies, serving as a conduit to meet key social determinates of health, such as housing, employment, food, clothing, and increased social supports for individuals disconnected from care or at points of care transitions. Drawing upon one's own personal recovery experience with mental health/addiction challenges, serves as a liaison across different care transitions to reduce avoidable higher levels of care, such as inpatient hospitalization and diversion from emergency department utilization, while improving the health outcomes, member experience and satisfaction.

CORE FUNCTIONS

1.Builds partnerships with individuals, families, youth, and key stakeholders to promote recovery, resiliency and wellness. Maintains a strong community presence to foster such ongoing partnerships both formally and informally.

2.Communicates and collaborate with members and families to identify concerns and remove barriers that affect service delivery or member satisfaction.

3.Offers member and family education and support to increase advocacy skills and engagement and promotes the development and use of member and family support programs.

4.Engages and communicates with a wide range of members in varied settings, performing reach-in activities with individuals to include but not limited to inpatient hospitals, crisis centers, emergency room departments, behavioral health residential facilities and justice system, assisting in engagement activities especially with individuals who are struggling to engage or maintain engaged in necessary services and supports.

5.Help individuals, through support, outreach and engagement activities establishing rapport to assist with getting an individual connected with a health care provider or system/s, so they can become empowered to take action to improve their own health status.

6.Through the identification of individuals with high utilization patterns, works aggressively with local network provider partners and community service agencies/peer organizations to provide targeted services aimed at engagement and activation of these individuals.

7.Leverages recovery-based tools and awareness of community services to assist member in establishing and achieving desired goals. Works as a short-term bridge supporting individuals from stepping-down from higher levels of care ensuring provider and community support/services connection.

8.Works to develop and utilize a comprehensive resource guide to inform and educate members and/or family members served about local community supports and resources.

9.Facility and community based while remaining within budgetary allowances. Internal customers:

BUHP Executive Management Team, other BUHP departments, Banner Health departments, Member Advocacy Council, Youth Leaders Subcommittee, Neighborhood Networks. External customers: AHCCCS, Families, Members, community agencies, provider networks, community advocacy groups such as: National Alliance on Mental Illness, Mental Health America of Arizona, Arizona Peer and Family Coalition, and others; Peer and Family Run Organizations, other stakeholder groups offering member and family voice.

MINIMUM QUALIFICATIONS

Associates degree in social work, public health or related human services field preferred; years of experience in the field may substitute for degree requirements.

Two plus years knowledge, skills and abilities obtained through work experience in the field of general mental health, substance use and recovery. Experience receiving Medicaid services and/or navigating a public health or social service delivery system or experience assisting a family member in navigating a publicly funded system.

Knowledge of existing community advocacy groups and ability to collaborate with multiple system stakeholders. Knowledge of community resources for housing or treatment to assist in recovery from mental or physical illness, and ability to work with existing resources to expand the system of care.

PREFERRED QUALIFICATIONS

Bachelor's degree in social work, public health or related human services field preferred; years of experience in the field may substitute for degree preference. Certified Psychiatric Rehabilitation Practitioner (CPRP) or Certified Peer/Recovery and/or Family Support Specialist preferred. Additional related education and/or experience preferred in the field of general mental health, substance use and recovery principles and practices.
