JOB TITLE: Independent Living Skills Case Manager

Immediate Supervisor: Independent Living Skills Program Manager

Supervision Exercised: N/A

SUMMARY: ILS is a community-based program that specializes in teaching adults with disabilities the fundamental skills needed to live independently. The ILS case manager provides support in skill development to prepare participants to move out on their own and live independently, while also providing supports for participants that are currently living alone but need assistance to maintain their independence.

DUTIES AND RESPONSIBILITIES:

  • Ensure highest ethical & business practices

  • Ensure compliance with all legal requirements and agency policies

  • Ensure the health and safety of people receiving services, personnel and visitors at all times

  • Ensure a healthy, safe, clean working environment

  • Develop and Implement participant's Individual Service Plans (ISP)

  • Develop participant's quarterly progress reports

  • Schedule and attend participant's quarterly, annual, and semi-annual meetings

  • Provide direct support to participants in learning their goals either in person or virtually

  • Build professional relationships with participants and stakeholders

  • Provide daily documentation of participant sessions, mileage reimbursements, etc

  • Timely completion of all required paperwork (ie case notes, grids, end of month billing)

  • Maintain monthly communication with individuals concerned with participant including family, service coordinator and others

  • Identify opportunities for increased independence

  • Other duties as assigned

  • Participate/Actively engage in opportunities for self-education, learning and development and ongoing all-staff and department-specific DEI trainings and dialogues

  • Participate/Actively engage in reflective practice and feedback as part of onboarding, 30-60-90, annual DEI goal setting, and performance reviews

  • Engage in feedback loops with managers, staff, teams, clients, and listen with the intention of understanding people's experiences

  • Collaborate with peers, community members and partners to incorporate DEI learnings about bias, microaggressions, discrimination, current social events and lived experiences (and any other DEI-related topics) to shape program services, self-care practices and organizational culture

GENERAL ADULT SERVICES RESPONSIBILITIES:

  • Comply with participant's rights, AbilityPath and program policies, CARF standards, NPS compliance, CCL regulations, CAL-OSHA, quality assurance standards, etc.

  • Must act as a Mandated Reporter, and responsible to report any known or suspected adult/child abuse

  • Must comply with applicable federal, state, and local regulations and laws

  • Participate in organization functions and attend staff meetings and trainings as scheduled

  • Partner with other AbilityPath departments to coordinate services and collaborate

QUALIFICATIONS FOR POSITION:

  • Ability to provide engaging instruction to participants with various developmental levels, skill sets, and behaviors

  • Ability to respond to challenging behaviors and support behavioral plans to increase engagement

  • Ability to work effectively on teams, including cross-departmental teams

  • Proficient in computer systems including Outlook, Excel, and Word

  • Ability to work with detailed and confidential information

  • Strong oral and written communication skills and ability to effectively communicate with parents, caregivers, team, and external partners

  • Demonstrate a positive outlook and ability to adapt to change

  • Passion for the mission, by believing in the abilities of people with disabilities

  • Awareness of, sensitivity to, and competence in supporting people with developmental disabilities

EDUCATION AND WORK EXPERIENCE:

Any combination of education and experience that would likely provide the required knowledge, skills and ability is qualifying, unless otherwise specified.

  • Associate's or Bachelor's Degree in relevant field

  • Experience working with adults with developmental disabilities

HIRING REQUIREMENTS:

  • Valid California Driver's License/ID

  • Clean Driving Record (if applicable)

  • Proof of negative TB test results

  • Verification of physical clearance

  • Passing a criminal fingerprint clearance

  • 18 years of age or job permit clearance

PHYSICAL ACTIVITY:

Accommodations made upon request to Human Resources

  • Ability to physically respond to adults with developmental disabilities

  • Ability to lift/support at least 40 lbs as needed

  • Ability to sit, stand, stoop, and bend frequently

  • Ability to walk for extended periods of time and support adults with developmental disabilities on community outings

  • Ability to drive company vehicles to transport participants

  • Ability to sit and keyboard as needed
