The IFPS Specialist provides very intensive strength-building services to children and families at imminent risk of disruption of out-of-home placement. Caseloads are kept very small and require a high degree of flexibility to schedule services around the needs of families served and to adjust to a constantly changing caseload. Services are family-centered, child-focused, and are delivered in the family's home (10-20 hours per week) for a period of 4 weeks. Services provided by the IFPS Specialist include assessment, service planning and implementation, individual and family counseling, budgeting, parenting skills training, life skills training, anger management counseling, home maker skill development, assistance with housing and educational needs, advocacy, referrals for service, etc. The IFPS Specialist is required to provide 24/7 on call as scheduled.


Responsible for providing direct social work services to each assigned client/client family as outlined by the IFPS program model under which the client is receiving services.

Area of Responsibility

  • Provide comprehensive client/client family assessment appropriate to the services and program model under which the client/client family is receiving services

  • Development of a comprehensive service plan and discharge plan based on client/client family strengths, needs and preferences

  • Ensure that the service planning, service plan revision and discharge planning process is meaningfully inclusive of the client, family, and other community/support persons unless otherwise documented/approved by Program Supervisor

  • Monitor the progress of each need area as identified in the service/person-centered plan and facilitate revisions as appropriate

  • Develop a relationship with the client/client family that facilitates achievement of their service plan goals

  • Provide and coordinate service interventions intended to stabilize and strengthen the individual client/client family system and eliminate the need for out-of-home placement, promote permanency, reduce delinquency or develop self-sufficiency as appropriate to the client and program under which they are receiving services. Interventions may include individual/family counseling, parenting education, life skills training, home maker services, information and referral, advocacy, and other support services delivered in the agencies office, in the client/family's home, in the client's school, or other community settings deemed appropriate and agreed upon by the client/family

  • Conduct aftercare follow-up contact with the client/client family as specified by the program service model

  • Coordinate and oversee initial and ongoing service assessment activities and perform all social work, case management and aftercare functions in a manner consistent with the applicable program/service model and with cultural competence

  • Review agency consumer satisfaction, outcomes and risk management data; follows supervisory suggestions for service modification and improvements based upon this data; and suggests ways services could be further improved in response to this data


  • Display sensitivity to the service population's cultural and socioeconomic characteristics

  • Perform other duties related to department goals and projects as needed


  • Minimum Bachelor's degree required

  • Educational and experience levels as determined by the Council on Accreditation, the NC Department of Health and Human Services licensing regulations for that position, or a combination of education/experience as determined appropriate by the agency

  • Effective verbal/writing skills

  • Maintain a valid North Carolina driver's license, adequate auto insurance, and have access to an operating vehicle in order to attend to CHS business
