Aveanna Healthcare, the largest home health care company in the U.S., is hiring an ID/DD Coordinator.
Description of Responsibilities
Responsible and accountable for the coordination and administration of services for clients and their caregivers.
Provide initial evaluation to determine appropriateness and safety of client and environment.
Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws.
Provide a written summary for individual clients and services based on needs, goals, and expected outcomes in conjunction with the ISP to Supports Coordinators/Administrative Entities on a monthly basis.
Participate in the development of Individual Support Plan.
Maintain patient confidentiality.
Demonstrate an understanding of client's medical status and updates self on clients/family status as needed or requested.
Maintain effective communication between staff, supports coordinators, and family.
Review progress notes from paraprofessionals to determine appropriate service delivery.
Educate paraprofessionals on individual client ISP expectations.
Perform on site evaluation of employee performance at 90 days and annually.
Other duties as assigned by supervisor.
Proof of eligibility to work in the United States.
Current CPR certification.
Bachelor's degree in Psychology or other social science or related field.
Current completion of child abuse clearance and Fingerprinting.
Good organizational and communication skills
Minimum of one year of experience with ID/DD preferred.
Occasional lifting and positioning of clients
Repeated or prolonged standing, bending, kneeling, twisting and occasional climbing of stairs.
Acute sense of sight and hearing (corrected or uncorrected) and strong sense of touch, smell and taste.
Agility and strength sufficient to handle clients and equipment without assistance.
Must be able to appropriately respond physically and psychologically to emergency situations in the home or during transport.
Must be able to function in a wide variety of environments which may involve exposure to allergens and other health conditions.
