Icmp Social Worker - Per Diem

The Integrated Care Management Program (iCMP) at MGH is looking for a dynamic clinical social worker with excellent clinical, care coordination experience with strong organizational skills.

The position is in a program of care coordination with a clinical social work focus, and this role is embedded in a primary care at MGH within a multidisciplinary team. The role consists of assessing patients' needs and providing strong collaboration within the organization and community resources to provide individualized clinical interventions to patients and their families to provide optimal outcomes.

Work expectations for employment in this role:

• Contributes coverage for leave of absence(s), sick coverage, and vacation coverage need for the iCMP social work team.

• Summer Vacation and days surrounding Holidays Coverage: June, July and August (summer) and around holidays for increased per diem hours.

• Contributes to enrollment, ongoing care coordination, mental health support, post-discharge assessment and other programmatic needs of the iCMP social work team.

• Hours: working hours are from 8:30 AM- 5:00 PM, hours are full or half days, agreed upon by SW manager before shifts begin.

• Work environment: remote work, in-office if urgent need.

The per diem SW will follow the hospital TIME KEEPING policy and follows the Time Policy and Procedures set forth from the iCMP.

Hospital/Department: Meets all mandatory hospital/department required training and competencies

• Completes MGH Annual Clinical Required Training Fiscal Year (Health Stream/Dec).

• PPD screening and compliant with occupational health requirements.

• Completes all iCMP specific required trainings and competencies.

Qualifications:

• Master's of Social Work (MSW) from an accredited program required.

• Require current Massachusetts Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Social Worker (LCSW), or must obtain MA Licensed Clinical Social Worker (LCSW) within 4 months of hire date.

• Relevant clinical experience preferred.
