A full-time position is now available with Liberty Healthcare Corporation serving the Commonwealth of Pennsylvania's Department of Aging Link to Aging and Disability Resources system.

Pennsylvania's Aging and Disability Resources Center (ADRC) Program, which is known as the PA Link to Aging and Disability Resources or PA Link, serves as the commonwealth's 'No Wrong Door/Single Point of Entry' into the long-term services and supports (LTSS) system.

The PA Link serves as an information and referral network that helps persons over the age of 60 and individuals with disabilities locate long term services and supports, provides referrals and assistance to simplify access to resources, improves coordination among human service agencies, and offers person-center planning.

In this role, you will be part of a team whose main function is to foster and maintain local partnerships and provide technical assistance, information and support to the aging and disability communities in Pennsylvania.

This is a 37.5 hour-per-week field-based position which will require you to regularly travel throughout an eighteen County region in Pennsylvania (travel estimated at 70%) - while working the remaining hours remotely from your home office.

The specific responsibilities of this position will include:

  • Building, developing, and growing business relationships vital to the success of the PA Link

  • Monitoring program objectives through review, evaluation, and analysis of program policy, guidelines and budgetary data/information - including providing guidance to Service Area Oversight Committees and Lead Coordinators in order to enhance Service Area performance and maintain compliance with program policy

  • Taking a lead role in network collaboration and partner development activities

  • Providing guidance to Service Areas on programing related activities

  • Assisting with Person-Centered Counseling (PCC) activities to facilitate informed decision making about long-term services and supports (LTSS)

  • Organizing and conducting trainings and presentations

  • Coordinating and facilitating internal and external meetings

  • Ensuring reimbursement standards for PCC are being met by reviewing and validating PCC consumer documentation in the Social Assistance Management System (SAMS)

  • Identifying and resolving issues and conflicts within the service areas

  • Tracking performance indicators and report outcomes for the service areas within the region

  • Assisting with development, standardization, implementation, and maintenance of PA Link policies and procedures as well as current or potential grant opportunities

  • Monitoring program objectives through review, evaluation, and analysis of program and budgetary data

  • Responding to inquiries/questions related to the PA Link

  • Analyzing service area budgets and quarterly activity reports for compliance

You're a good fit for this position if you possess the following attributes:

  • Prior experience in healthcare / human services

  • Knowledge of services and supports for older adults and/or people who have disabilities

  • Ability to develop and maintain positive working relationships with a variety of stakeholders

  • Ability to work with a high degree of independence and autonomy

  • Outstanding oral and written communication skills

  • Proficiency with Microsoft Excel, PowerPoint, Word and Outlook

  • High degree of financial acumen and responsibility

  • Willingness and ability to travel extensively throughout Pennsylvania (approximately 50%)

An Associate, Bachelor's or Master's degree in Social Work, Psychology, Education, Sociology, Criminal Justice or other human services / healthcare field is preferred. Working knowledge of PA Link, PCC, LTSS and/or SAMS is highly desirable.

Due to the travel requirements associated with this position - applicants should have a PA driver's license as well as an insured vehicle.
