Your Specific Responsibilities:

This position spends more than 50% of the time engaged in supervisory responsibilities. This includes communicating with, motivating, training, and evaluating employees in addition to planning and directing employees' work. The incumbent has the authority to hire, transfer, suspend, lay off, recall, promote, discharge, assign, reward, or discipline subordinate employees or effectively recommend such action. The incumbent will also be required to develop performance expectations, meet with staff regularly to discuss performance, complete performance reviews, maintain vacation and work schedules, ensuring proper coverage, and complete timesheets.

This is a highly responsible position involving PHAB Accreditation, Community Health Improvement activities, Quality Improvement activities Healthiest Weight Florida, Heart Health Plus, and other chronic disease prevention activities, including special projects as funded. The incumbent will need to be able to work independently and as part of a team, prepare and submit funding applications, and perform budget management responsibilities for Healthiest Weight Florida, Heart Health Plus and Tobacco Programs.

Chronic Disease Program Management

  • Responsible for the implementation of the Healthiest Weight Florida, and Heart Health Plus annual work plans. Implement and complete all work plan deliverables by the specified due dates. Develop and implement chronic disease prevention initiatives and projects as directed, based on funding allocations. Manage associated work plans and budget tasks, as assigned by supervisor. Obtain necessary training and establish an ongoing diabetes prevention program and an ongoing diabetes self-management education program.

Provides Supervision to Employees. Provides supervision to three Tobacco Program Staff and one Chronic Disease Staff. This includes four FTES. Incumbent is responsible for promoting a tobacco free lifestyle to support staff and their programs in the communities.

Community Health Improvement and Accreditation

  • Assist in conducting, implementing, and facilitating community health assessments, data analysis and presentations, and community meetings. Ensures that Community Health Improvement activities are implemented within established timelines. Assists with all accreditation activities as needed. Produces meeting minutes, progress reports, and other required documentation.

Heart Health Plus Statewide Training Coordinator

  • Participates in CDC training sessions on behalf of the State Health Office Chronic Disease Program. Provides CDC approved training to county Heart Health Plus Coordinators and assists with local implementation.

Level 4 Budget Manager. Performs Level 4 Budget Management activities associated with Healthiest Weight Florida and Heart Health Plus. This includes all purchasing, reconciling and balance projections, with the assistance of the Budget Manager. Meets with the Budget Manager to ensure that the FIRS budget module correctly reflects the new fiscal year budget and reconciles budgets prior to June 30.

Performs other related duties as assigned.

Qualifications:

  • Bachelor's Degree or Nursing license
