REQUIREMENTS

Incumbent must be a person living with HIV or have personal experience with PrEP. Call clients with appointment reminders for eligibility, ADAP, case management, doctor and lab appointments.

Meet with new clients as the enter or re-enter the Area 3/13 County Health Department's specialty care clinics or PrEP clinics. Follow-up with clients to overcome barriers to care and remain in care. Work on linkage to care program.

Assist clients with adherence. Accompany/transport clients to appointments if available and requested. Complete forms and documentation accurately and as instructed. communicate with CHD clinic staff, Ryan White eligibility and case management staff as needed and directed.

Attend meeting and clinics as directed. Participate in outreach activities as requested and as schedule permits.

This position requires that the candidate have a valid driver's license and access to reliable transportation.
