The work location of this position is in Acton, MA. This is a blended caseload of Autism Spectrum Disorder only and Intellectual Disability consumers.

Prior experience providing supports for individuals with Autism Spectrum Disorder is required due to extensive Autism related needs and services comprising the caseload for this position.

The Department of Developmental Services (DDS) is dedicated to creating, in partnership with others, innovative and genuine opportunities for individuals with intellectual disabilities to participate fully and meaningfully in, and contribute to, their communities as valued members. DDS is seeking motivated and experienced candidates for the role of Human Service Coordinator A/B.

The Human Services Coordinator A/B will coordinate and facilitate the development of the (ISP) Individual Service Plan for individuals on caseload within established timeframes. He or she will arrange and organize the Dept. of Developmental Services (DDS) funded and generic support services in response to individual's needs.

The selected candidate will monitor the implementation on these support services and ISP process and monitor DDS funded residential and day program supports-visiting on a frequent basis and documenting these visits. The Human Services Coordinator A/B will monitor the safety and well-being of assigned individuals and provide advocacy in human, civil and legal rights. He or she will maintain communication and outreach with community services and other state agencies and coordinate referrals as needed. The Service Coordinator will maintain individual records and relevant computer databases for assigned individuals as well as maintain regular and consistent contact with individuals, service providers and families as indicated. The incumbent will arrange for and provider crisis intervention for individuals as needed-assessing needs and making recommendations for follow up planning. He or she will facilitate team meetings and monitor follow-through on recommendations.

The Human Services Coordinator will provide information to supervisor and other members of the Area Office Management Team as needed and represent Area Office effectively. The Service Coordinator will also perform other related duties as required. Interested applicants should be flexible in schedule as there may be occasional nights and weekend meetings with individuals, guardians, families, and other agencies.

A criminal background check (CORI) will be completed on the recommended candidate as required by the regulations set forth by the Executive Office of Health and Human Services prior to the candidate being hired. In addition, Chapter 234 of the Acts of 2014, An Act Requiring National Background Checks, requires the Department of Developmental Services to conduct fingerprint-based check of the state and national criminal history databases on the recommended candidate. All Commonwealth employees are paid through Direct Deposit on a Bi-Weekly basis. Listed Salary ranges are for annual compensation for 1 full year at current rates.

Schedule: Full-Time, 37.50 hours per week
8:45 AM-5:00 PM Monday-Friday

Days Off- Saturday/Sunday

MINIMUM ENTRANCE REQUIREMENTS:

REQUIRED WORK EXPERIENCE: At least three years of full-time, or equivalent part-time, professional experience in human services work or social work. Based on assignment, one year of professional work experience must have been in one of the following specialty areas. Refer to the individual position description for specialized work experience required.

  • Mental Health Case Manager and Forensic Transition Team Coordinator assignments only: The year of professional work experience must have included the clinical care and treatment of psychiatric or emotionally disturbed individuals.

  • Department of Developmental Services assignments only: The year of professional work experience must have involved working with individuals with intellectual or other developmental disabilities.

SUBSTITUTIONS:

  • A Bachelor's or higher degree in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, or physical education may be substituted for two years of the required experience on the basis of two years of education for one year of experience.

  • Mental Health Case Manager and Forensic Transition Team Coordinator assignments only: Degree substitutions must be in social work, psychology, sociology, counseling, counseling education, rehabilitation, rehabilitation counseling, human services, education of the physically or emotionally handicapped, education of the multiple handicapped, or education of the learning disabled.

  • Department of Developmental Services assignments only:

  • A current and valid Massachusetts license or registration as a physical therapist, speech-language pathologist, occupational therapist, audiologist, registered nurse, dietitian, or social worker may be substituted for two years of the required experience.

  • A current and valid Massachusetts license or registration as a physical therapy assistant or occupational therapy assistant may be substituted for one year of the required experience.

  • One year of education equals 30 semester hours. Education toward a degree will be prorated on the basis of the proportion of the requirements actually completed.

  • No substitutions will be permitted for the required year of professional experience.

REQUIRED EDUCATION AND LICENSES:

  • One of the following is required for Qualified Developmental Services Professional assignments in the Department of Developmental Services only:

  • A current and valid Massachusetts license or registration as a physical therapist, speech-language pathologist, occupational therapist, audiologist, registered nurse, dietitian, or social worker, OR

  • A Bachelor's or higher degree in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, or physical education.

  • Based on assignment, travel may be required. Those employees who elect to use a motor vehicle for travel must have a current and valid Massachusetts Class D Motor Vehicle Operator's license or the equivalent from another state.
