Provides comprehensive case management of long term care services for at-risk, fragile elders both within their homes and at the City's four Seniors Program Centers.

Position involves competing demands, performing multiple tasks, working to deadlines, frequent work at night, beyond normal business hours. Regular attendance is an essential function of this job to ensure continuity of services. Position is subject to drug testing in accordance with applicable State and Federal regulations and City of Las Cruces policies.

Work is performed in a standard office environment, client homes, and in various community facilities. Light physical demands; some lifting and moving of supplies and equipment.

Full-time, regular, exempt

Duties and Responsibilities:

  • Conducts in-home and center-based interviews with English and Spanish speaking clients in order to provide services to protect the health and safety of senior clients, in compliance with state and Federal regulations; determines client eligibility, determines services needed, determines program eligibility, and assigns services; monitors client progress at least every three to six months, and manages a caseload; develops and implements client care plans,

  • Maintains regular communication with internal staff, family members, and professional agencies in order to access additional community services; conducts field visits and makes visual assessments of client's physical surroundings to assess hazards to health and safety; oversees the management of client services of various subcontracting agencies in accordance with federal and state regulations and stipulations of the contracts, grants and agreements.

  • Maintains all client documentation, including written assessment tools, case notes, family correspondence, referrals, legal reports and documents; uses a computerized client data base in order to assure necessary assessments and services were arranged and provided; coordinates information and referrals with other social service agencies and health care providers in order to obtain necessary services to protect both health and safety.

  • Maintains on-going community networking efforts with home health care agencies, hospitals, doctors, and public and private social service agencies to assure all possible services are provided to clients; develops and coordinates special projects for in-home services including planning, promoting, implementing, and evaluating special activities and events.

  • Assists with the development and coordination of training for the In-Home Services unit; participates in special events, activities and programs sponsored by the Senior Programs Department.

Minimum Qualifications:

Bachelor's degree in Social Work, Nursing, or related field AND two years of experience in casework management and community service programs for the elderly and disabled communities; OR a combination of education, experience, and training may be applied in accordance with City of Las Cruces policy.

Licenses/Certification(s)

Valid driver's license is required. Position requires an acceptable driving record in accordance with City policy. Bilingual (English-Spanish) skills preferred. Case Management Certification is desired.

Knowledge, Skills, and Abilities:

Knowledge of: Elder care issues, especially with high risk clients; programs that provide social services in the community; case management process for Long Term Care services including but not limited to eligibility for State and Federal benefits and insurance, DD Waiver, D & E Waiver, SSI and SSDI, Medicare Part A & B, Medicaid, Medically Fragile, Durable Medical Equipment, PC Option, and other community benefits; policies and regulations governing eligibility for clients; effective interviewing techniques and procedures; principles of record keeping and records management; business and personal computers, and standard business software applications; City policies and procedures.

Ability to: Assess and prioritize multiple tasks, projects and demands; communicate effectively in verbal and written forms; maintain confidentiality of program records and information; establish and maintain effective working relations with employees and the general public; carrying out primary responsibilities in accordance with all governing regulations, statutes, and procedures.

Skills in: Interpreting social service program regulations in a clear, accurate, and professional manner to clients; listening, assessment, treatment planning, monitoring and problem solving; case management record keeping, and professional/technical reading and writing; working effectively with a diverse cultural community.
