What You Will Do to Change Lives

The Housing Coordinator plans, compiles, directs, and coordinates housing related activities to include but not limited to: establishing eligibility, completing all associated applications and paperwork required for voucher attainment and leasing, certification, re-certifications, and housing inspections for approximately 50 clients.

Shifts Available:

Full Time 40 hours per week; Monday

  • Friday; 8:00am

  • 4:30pm IN PERSON POSITION

Expected starting wage range is $66,560.00 - $70,047.38. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

POSITION SUMMARY

The Housing Coordinator plans, compiles, directs, and coordinates housing related activities to include but not limited to: establishing eligibility, completing all associated applications and paperwork required for voucher attainment and leasing, certification, re-certifications, and housing inspections for approximately 50 clients.

QUALIFICATIONS

Required:

  • Bachelor's Degree and one year of experience providing housing services to clients or Associates Degree in social services field and three (3) years of experience providing housing related services to behavioral health clients or five (5) years of experience providing housing related services to behavioral health clients

  • Must understand and apply of Federal, State, and local housing rules and regulations

  • Knowledgeable of cultural and socio-economic factors that inform service provision to persons with behavioral health issues

  • Must have personal car, willing to use for work and transport clients to appointments

  • Must have valid CA Driver's License in good standing

ABOUT THE PROGRAM

Program Type: Community-based

To Make Referrals: A referral to CARE Court can be initiated by family members, behavioral health providers, first responders, or other approved petitioners by filing a petition with the Superior Court. Referrals to Telecare CARE ACT are made by the County of San Diego.

Who We Serve: All criteria must be met to be eligible for admittance

  • Aged 18 years and older.

  • Diagnosed with a schizophrenia spectrum disorder or another psychotic disorder in the same class.

  • Individuals must be experiencing symptoms that are persistent, interfere with daily life, and may lead to a loss of independence.

  • Individuals can not receive voluntary treatment elsewhere.

  • One of the following must be applicable:

  • Individual is at risk of safely interacting with the community without supervision, and their condition is deteriorating.

  • Individuals who need services to prevent relapse or deterioration that would result in disability or serious injury to themselves or others.

  • Individuals who will likely benefit from ACT treatment plan.

Exclusion Criteria:

  • Individuals with a serious mental illness, such as bipolar disorder or major depression, are not eligible.

  • Mental illness must not be based on a medical condition, including a physical health condition such as a traumatic brain injury, autism, dementia, or a neurological condition.

  • Individuals with a current diagnosis of substance use disorder must also have a psychotic disorder and meet all the other criteria to be eligible.

ESSENTIAL FUNCTIONS

  • Demonstrates the Telecare mission, purpose, values and beliefs in everyday language and contact with the internal and external stakeholders

  • Must implement and effectively manage potentially dangerous or crisis situations by implementing CPI strategies of de- escalation and evasion as appropriate

  • Conducts screening, engagement, establishes eligibility for housing program, obtains necessary documents, and assists eligible clients to obtain affordable housing in accordance with Federal, State and Local housing regulations, forms and databases (e.g., VISPIDAT, CAHP, Service Point)

  • Assists clients with Section 8 as well as other available housing vouchers and resources paperwork

  • Locates a full continuum of housing opportunities within the defined areas of the County and matches clients with appropriate housing placements that accept the sponsor-based subsidy

  • Travels throughout the County performing fieldwork and transports clients to visit permanent housing locations and interviews and interacts with vendors to ensure housing payments are made in a timely manner

  • Inspects housing units to ensure compliance with established housing standards, assists clients with meeting and maintaining these standards and coordinates required housing inspections

  • Provides client advocacy with prospective/current landlords and community supports and educates them to the program

  • Provides education workshops for clients on-site, or in the community, to inform clients of information regarding their housing options at least 3 times per fiscal year

  • Works closely as a fully integrated member of the ACT team to ensure client needs are being addressed to assist in gaining and maintaining housing goals

  • Attends all required provider meetings, acts a liaison with members of the housing industry and other providers

  • Provides administrative and programmatic support for the successful operation of the housing subsidy program

  • Must utilize all available housing subsidies to house clients who are homeless

  • Must manage housings vouchers

  • Must maintain access to an array of housing options to include short-term, transitional or permanent supportive housing that may include housing subsidies, in accordance with Title 9, Housing Urban Development (HUD)

  • Must ensure non-institutionalized enrolled clients are housed in short-term, transitional, or permanent supportive housing

  • Must work with housing technical expert to identify a range of housing opportunities for homeless clients, including but not limited to short-term stays, transitional housing, shared housing, independent living homes, and permanent housing

  • Must manage programs subsidizes per policy

  • Must support all potentially eligible clients in applying for Housing Choice Voucher Program (Section 8) subsidies with local Housing Commission, and other Housing Authorities in the region. Position must ensure clients have access to permanent supportive housing subsidies administrated by local Housing Authorities

  • Must assure members eligible for Housing Authorities subsidies apply, complete eligibility process and be leased up within guidelines

  • If employed at Inpatient program, must assist with restraint of members served in the event of assaultive behavior and pass assault crisis/crisis prevention training

  • If employed at Outpatient program, must evade members served in the event of assaultive behavior and pass assault crisis/crisis prevention training

SKILLS

  • Familiarity with psychiatric conditions and recovery principles

  • Understand, interpret, compile, analyze and apply information for compliance with contract expectations and rules and regulations

  • Strong organizational skills

  • Possesses knowledge of landlord and tenant rights and responsibilities, housing quality standards and housing inspection techniques

  • Must have knowledge of an array of housing options to include short-term, transitional or permanent supportive housing that may include housing subsidies, in accordance with Title 9, Housing Urban Development (HUD)

  • Must be knowledgeable of community resources and coordinate client care with a variety of community-based services

  • Highly flexible with the ability to respond to and meet multiple demands and deadlines

  • Excellent customer service skills with demonstrated ability to interact well with others and function collaboratively in a team environment
