PROGRAM DESCRIPTION:

Funded by the NYC Department of Homeless Services, the Boulevard is a 101-bed homeless shelter that houses mentally ill men. The program provides a complete range of mental health stabilization, medication management, case management and medical services. The ultimate goal of the program is to place its clients into appropriate, supportive housing within nine months.

HOURS:

Full-time, 37.5 hours per week

Monday, Wednesday, Thursday, Friday; 9:00a

  • 5:30p, Tuesday; 11:00a

  • 7:30p

DUTIES/RESPONSIBILITIES:

Obtain supportive housing for clients, Carry 5- 10 case load of housing movable clients. Escort clients to medical, benefits, and housing-related appointments.

Compile, copy, and distribute HRA2010e housing packets; and complete corresponding consent forms. Issue residency letters and client appointment reminders. Complete various reports required by DHS.

Monitor and update all clients in AWARDS and CARES databases as well as caseload distribution report. Enter progress notes in both systems as well as ILPs in CARES as needed. Facilitate psychoeducational groups with clients.

Provide emergency first aid/CPR assistance when needed. Related duties as assigned.

QUALIFICATIONS:

High School Diploma/ GED required. BA preferred, Bilingual (Spanish-English) preferred.

Experience working with homeless mentally ill. Outreach, entitlement, and, substance abuse experience helpful. CASAC preferred.

Proficiency in MS Office, including Word, and Excel. Valid NYS Driver's License preferred. CPR training certification or willingness to take training class in CPR.

VIEW JOBS 4/23/2019 12:00:00 AM 2019-07-22T00:00 <p>Riis Settlement Senior Services NNORC is looking for a part-time NNORC Case Manager for five hours a day for 25 hours a week. The Case Manager will provide seniors with Case Management services to improve the quality of life for Queensbridge adults, aged 60 and over, to allow them to live healthier and safer lives in their own community and to successfully age in place. These services include assessment and re-assessment, care planning, arranging for services, follow up and monitoring. It may also involve counseling and advocacy, supportive contacts, and help with paperwork and applications for benefits and entitlements. In addition, home visits will be routine and necessary. The NNORC Case Manager develops and utilizes relationships with service providers and referral sources and engages in shared case management with Case Manager to facilitate the management of clients for NNORC programs. The NNORC Case Manager will join our NNORC team, (Neighborhood Naturally Occurring Retirement Community) a program in expansion dedicated to maintaining and improving the quality of life of Seniors (60 years and older) in the Queensbridge Houses and in Northwestern Queens.</p><p><strong>Primary Job Responsibilities:</strong><br></p><ul> <li>Connect with client and community resources by liaising with Senior Services staff to ensure the receipt of referrals and confirmation of client appointments</li> <li>Collaborate with client and all partners including the client’s family, health care providers and community partners to assess needs, develop a care plan, link the clients to available resources, and monitor and adjust plan as needed.</li> <li>Engage in shared case management with on-site nurse, and accompany on site- Nurse on client initial and ongoing visits</li> <li>Coordinate and monitor all aspects of support groups</li> <li>Manage daily, weekly and/or monthly case reports as needed for each client. </li> <li>Work with client and their families when communicating with other agencies</li> <li>Document all direct and collateral contacts and manage update of case files. </li> <li>Follow all documentation procedures for program and ensure that all case notes and charts are complete and up to date for quality assurance purposes.</li> <li>Review all of their charts bi-annually for quality assurance purposes. </li> <li>Participate in agency based activities which include: meetings, outreach efforts, fundraising, case manager trainings, and special projects as requested by the Director of Riis Settlement Senior Service NNORC</li> <li>Assist with additional program components as needed</li> </ul><p><strong>Requirements</strong></p><p><strong>Qualifications</strong></p> <ul> <li>Masters Degree in Social Work preferred will consider a Bachelor’s Degree in Social Work, Human Services or relevant degree, and three years of related experience</li> <li>Strength-based training such as Family Development Credential</li> <li>Experience working with Senior Citizens or demonstrated knowledge of the unique needs of Senior Citizens</li> <li>Experience working within a non-profit, community based organizations.</li> <li>Strength based- training such as Family Development Credential</li> <li>Experience working with Senior Citizens or knowledge of the unique needs of Senior Citizens</li> <li>Demonstrated experience in providing comprehensive case management services to large caseload of targeted population </li> <li>Experience and/or demonstrated comfort level working and conducting home visits in a New York City Housing Authority (NYCHA) public housing development. </li> </ul> <p></p> <p><strong>Statement of Skills, Knowledge &amp; Abilities:</strong></p> <ul> <li>Ability to organize and prioritize efficiently</li> <li>Able to Multi-task</li> <li>Strong interpersonal skills</li> <li>Strong communication skills</li> <li>Professional presence</li> <li>Knowledge of Entitlements, Medicaid, Medicare &amp; Managed Care </li> <li>Ability to work independently and in a team </li> </ul> Jacob A.
