Job Title: Housing case Manager
Reports to: Housing Program Manager
Location:Administrative office 202 se 181st Ave hybrid Remote
Status: Full Time 40 hours per week
Salary/Wage: $20.00- 22.00 per hour
The Housing Case Manager for the Ujamaa Enrichement program promotes housing stability and community connection for people referred to or signs up for programming throught Black Community of Portland. The case manager will provide hybrid-site support for 5-8 residents to connect to mental health, employment, and other needed services. This position emphasizes outreach-based services with a focus on individualized support, harm reduction, and connection to community. The right person for this role is highly engaging, creative, has knowledge of community resources, has strong boundaries, and a commitment to helping vulnerable people succeed in housing.
Requirements
Specific Responsibilities
- Provide outreach-based support to people assigned to you
- Collaborate with participants to assess service needs including mental health, income development, tenant education, and basic needs.
- Identify and coordinate connection to needed community providers.
- Meet with participants as needed to provide and coordinate delivery of services.
- Participate in screening and staffing meetings as needed with community partners
- Coordinate with landlords and property owners to mitigate tenant issues and advocate for participant needs.
- Coordinate with agency programs and administration to provide consistent quality and comprehensive services to participants.
- Develop and maintain relations with community resources to promote access and services for participants.
- Attend staff meetings, case management meetings and partner meetings as assigned.
- Maintain accurate, updated, and comprehensive participant files.
- Maintain accurate and updated documentation of services in HMIS.
- Adhere to allocated budgetary guidelines.
- Provide monthly Case Management Reports as requested.
- Other duties as assigned.
Performance Requirements
- Acquire First Aid/CPR/AED certification within 90 days of hire and be retested biannually thereafter throughout employment.
- Complete Assertive Engagement Training when designated to attend.
- Demonstrate reliability by being present for work as scheduled, arriving and leaving on time and taking breaks in expected time frames.
- Interact effectively with all staff, clients and community members and treat each individual with respect and dignity.
- Attend organization and program staff meetings.
Minimum Qualifications
- 2 years minimum of experience in social services or organizational work
- Demonstrate ability to interact with other agencies in developing and providing services to participants; including ability to advocate on behalf of participants for services with other agencies.
- Ability to identify and work effectively with property and building managers, developing positive long-term relationships.
- Ability to provide mediation between agencies, participants, and property and building managers.
- Up-to-date familiarity of community resources and services available to assist the homeless.
- Demonstrated knowledge of domestic violence and sexual assault issues and resources.
- Demonstrated knowledge of services available to assist people experiencing homelessness or housing insecurity.
- Demonstrated knowledge of services available to assist culturally specific clients and community members.
- Demonstrated ability to work as part of a team in the delivery of services to participants.
- Computer literacy. Demonstrated ability to use word processing, database, spreadsheet, e-mail, and Internet programs.
- Demonstrate ability to maintain accurate and comprehensive participant files.
- Demonstrate ability to prepare accurate and comprehensive statistical and narrative reports.
- Valid driver’s license, liability insurability and possession of reliable transportation, or otherwise capable of responsive off-premises mobility.
