POSITION SUMMARY:

Scheduled Hours per Day: Monday-Friday 8:30am-5pm

Under the direction of the Corporate Director of Housing Services the Housing Full Services Partnership (FSP) support staff provides client move in support; pick up, inventory and delivery of donations, repair assessment of Master Lease housing sites, purchasing of household supplies, assists with all housing social events and all other supportive services as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Encourages and promotes an environment that is strength based to assist clients in meeting their individual goals.

  • Outreaches and networks with food banks, community/private donors and Non-profit service providers within Los Angeles County to create ongoing donations (i.e. shelter partnership, furniture, appliances, food etc.) and maintains a listing of all donors.

  • Coordinates pick up, delivery and distribution of all donated household, cleaning, office and hygiene supplies for all housing sites.

  • Completes monthly inventory and distribution reports and submits to direct supervisor.

  • Provides consumers with move in assistance by coordinating delivery of needed appliances, furniture and personal belongings to new housing placement.

  • Conducts master lease property site inspections, troubleshoots/assesses needed repairs, and submits repair request forms for approval to Housing Department designee in a timely manner and within appropriate time frames, schedules and coordinates repairs to maintain property beautification.

  • Conducts housing search, pre-inspections, quarterly site visits of SPC/Section 8 units to ensure Housing and Urban Development (HUD) compliance.

  • Reports all client mental health issues to Treatment/Housing teams in a timely manner, to troubleshoot and coordinate appropriate supportive services for clients and program benefit.

  • Assists with the coordination of program Social Events, such as set up/tear down, clean up and transportation of necessary supplies.

  • Completes and submits reports and documentation expectations within the timelines established by the Program for service delivery per Agency requirements and provides services consistent with program needs pursuant to funding source directives, Pacific Clinics' protocol and Health Insurance Portability and Accountability Act (HIPAA) compliance requirements.

  • Drives Agency-provided or personal vehicles for consumers, staff and others, as needed.

  • Attends and participates in staff meetings to provide input towards program development and staff training.

  • Interacts with other internal programs, clinical/support staff and external sources such as school or community personnel as applicable.

  • Reports to work on time and maintains reliable and regular attendance.

  • Models Pacific Clinics' approach, mission and core values in all communication and correspondence.

  • Communicates effectively in a culturally competent and diverse consumer population and promotes favorable interaction with managers, co-workers and others.

  • Performs other duties as assigned

QUALIFICATIONS:

ORGANIZATIONAL RELATIONSHIPS/INTERACTION:

Initiate and maintain professional interactions and communication with Clinic's employees and/or others.

EDUCATION and/or EXPERIENCE/POSITION REQUIREMENTS:

  • High school diploma/GED and/or one (1) year of related experience in the areas of customer service and/or shipping and receiving preferred.

  • Demonstrated experience and ability to relate to a culturally diverse clientele and staff.

  • Bilingual skills in the appropriate language for the Clinics' clients are highly preferred.

  • Must demonstrate ability to work as part of a multidisciplinary team.

  • Must be able to work under minimal supervision.

  • Possess strong community outreach skills.

  • Communicate effectively both verbally and in writing.

  • Strong paperwork and documentation skills.

  • Establish and maintain effective working relationships with coworkers, clients and community agencies.

  • Effectively assist with housing activities in multiple service sites both within the agency and in the community.

  • Must possess a valid California driver's license and maintain an insurable driving record under the Clinics' liability policy.

PHYSICAL DEMANDS:

While performing the duties of this job the employee is frequently required to stand or sit. The employee is required to use hands to produce records and/or documentation in manual or electronic format. The employee must be able to life and/or move items up to 20 lbs., and periodically up to 50 lbs.

While performing the duties of this job, the employee is occasionally exposed to moving mechanical parts and outside weather conditions. The noise level in the work environment is usually moderate. Community outreach and local business travel is required.
