Job Summary

Performs light cleaning duties to maintain establishments, including hotels, restaurants and hospitals, in a clean and orderly manner. Duties include making beds, replenishing linens, cleaning rooms and halls, and vacuuming.

Essential Duties and Responsibilities:

  • Provides quality customer service to customers by providing one-on-one attention to detail.

  • Sweeps, scrubs, mops and polishes floors.

  • Vacuums carpets, rugs and draperies.

  • Shampoos carpets, rugs and upholstery.

  • Dusts and polishes furniture and fittings.

  • Cleans metal fixtures and fittings.

  • Empties and cleans trash containers.

  • Disposes of trash in a sanitary manner.

  • Cleans wash basins, mirrors, tubs and showers.

  • Wipes down glass surfaces.

  • Makes up beds and changes linens as required.

  • Realigns furniture and amenities according to prescribed layout.

  • Responds to guest queries and requests.

  • Responds to calls for housekeeping problems, such as spills and broken glasses.

  • Contributes to team efforts; exhibits professionalism with customers, fellow associates and others.

  • Performs other duties as assigned.

Responsible for assessing and providing consultation to determine qualification for home health and home infusion services and coordinating these services for patients being discharged from the for MedStar hospitals. Responsible for building and maintaining positive customer relations with referral sources and key stakeholders at the MedStar hospitals. Generates accurate referrals utilizing the most efficient processes and tools available in MedStar. Contributes to creating an environment of teamwork; develops trust with co-workers and with other Agency staff. * Minimum Qualifications * Education/Training * Valid clinical license; Bachelors degree preferred, or an equivalent combination of education and experience. * * Experience * 2 years acute care or related experience; 1 year case management / homecare experience preferred. * * License/Certification/Registration * Valid RN, PT or OT professional license in the District of Columbia, Maryland or Virginia, or any combination as required based on work location(s). * * Knowledge, Skills & Abilities * Strong organizational skills and ability to work under deadlines. Self-directed team player. Basic computer skills. Strong interpersonal skills, including verbal and written communication. Ability to communicate with patients, their relatives, physicians and a variety of personnel. * Primary Duties and Responsibilities * Care Coordination: Collaborates with the hospital clinical team to develop and process home health referrals. This includes, but not limited to, offering patient choice, educating patients/families on what to expect from home health services and arranging these services. * Clinically assesses patients to coordinate their home care services. * Contributes to the achievement of established goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Completes annual Code of Conduct, Compliance and other mandatory training. Complies with governmental and accreditation regulations. * Customer Service: Seeks to build new and maintains effective working relationships with referral sources, key stakeholders and other departments. Participates in meetings and on committees supporting the Department and Agency marketing and outreach efforts. * Educates patients on home care services and disease state and processes as applicable. * Expert Resource: Serves as expert resource to referral sources including case managers, physicians, and home care operations on home health services and practices as well as the home health referral process. Provides education to referral sources, providers, members of the clinical team regarding home health and infusion services. * Performs consultation on all high risk patients to determine whether patients meet criteria/qualifications to receive home health or home infusion services. * Performs other duties as assigned. * Productivity/Budget: Achieves or exceeds budgeted VNA/MHI admissions through effective referral development. Processes on average a minimum of 80 referrals per month. * Professional Development: Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations. * Team Work: Collaborates with the Home Care Coordination Team in the daily routines of referral development and home care coordination including monitoring daily census, keeping detailed daily logs/statistics and keeping work load organized and prioritized. Consults with and supports referral sources and home health and home infusion agency staff on complex cases. * Technology/Workflow: Utilizes technology to effectively maximize efficiency, and improve accuracy and communication of daily care coordination activities. Documents care coordination activities in hospital case management and MedStar My Health plan electronic data base. Ensures appropriated information is included in patients medical record. Is proactive in developing workflow practices that support Agency needs.
