Position Summary
First and foremost, the manager position is responsible for the delivery of quality consume care. This position requires discretion and independent judgement, supervising staff (personal care technician/advance, C.N.A, behavioral care technician, etc.) in the homes and reporting to Senior Case Manager under the general supervision of the Program Manager. Managers work with nursing and behavioral health support staff to coordinate and monitor the treatment planning process and ensure each consumer has a safe, healthy, and stable home to live in.
Case management combines the skills of direct care and professional management. All managers must be skilled in maintaining records and charts, preparing written reports, performing and providing training on direct care skills, and maintain effective working relationships with others. Managers also serve as advocates on behalf of our consumers with medical, transportation, and other professionals in the service system. This requires managers to take a hands-on approach to their job so that they know the consumers' daily schedules, support plans, behavioral plans, and medical needs.
This is the entry level position. At this level, the focus is on learning the procedures, practices, and techniques for the essential duties described above as well as developing supervisory oversight and gaining competency through both formal and on-the-job training. Caseloads are typically lighter, between 1-2 homes/clients and monitored by a Senior Case Manager. Positions continue to develop knowledge of consumer case management activities and associated concepts, principles, practices, and techniques. Work is performed under close progressing to limited supervision.
Essential Functions
1.Maintain a safe consumer environment at all times
2.Supervise Personal Care Technician staff providing services to each consumer, ensuring proper training, disciplinary action, and performance management
3.Train and work closely with Personal Care Technician-Advanced, delegating back up case management tasks as appropriate
4.Follow Individual Support Plans (ISPs) and /or Behavioral Support Plans (BSPs) as directed
5.Use proper techniques in performing direct care in accordance with company policy and procedures, accepted standards of care, and applicable regulations and requirements
6.Assist with and train staff on basic daily living skills such as meal planning and preparation, maintaining a clean environment (laundry, household cleaning, etc.)
7.Regularly visit all consumer homes to evaluate standard of care and perform surprise check-ins of Personal Care Technician staff, to include on-site supervision as needed
8.Communicate the consumer care plan with applicable car team members, remaining aware of consumer needs and changes in consumer's condition
9.Manage house finances, including FBO accounts and spending logs
Monitor correctness and timely completion of daily logs and other reports, ensuring records are accurate, clear, and up to date
Review all documentation completed by Personal Care Technician staff to ensure appropriate intervention based on plan of care; monitors accuracy of documentation
Complete monthly schedule in accordance with company policy
Review and approve staff timecards and employee time off (ETO) requests
Work with Residential Coordinator and Residential Services Manager to ensure proper setup, certification, and maintenance of consumer homes
Coordinate team efforts preceding and after any crisis or situation that causes interruption of care
Ensure proper completion of incident reports in a timely and accurate manner
Complete change of status forms, workers compensation forms, and other HR forms as necessary
Serve as consumer advocate, assessing care and ensuring that quality care is given
Ensure that equipment used by the team is well maintained, used appropriately, tracked and returned
Evaluate the effectiveness and outcome of care
Plan for consumer discharge when necessary
Participate in on-call as per agency protocol and schedule

Supervisory Responsibility
This position manages all employees of the assigned department(s) and is responsible for the performance management and hiring of the employees within that department.
Required Education and Experience
1.High School Diploma or GED
2.Experience in direct care preferred but not required
Preferred Education and Experience
1.Bachelor's Degree
2.At least two years' experience in a direct care role
3.Prior supervisory or management experience
4.For medical homes: CAN or other medical degree (or 2+ equivalent years of experience as medical manager)
5.For behavioral homes: extensive experience/background working with challenging behaviors
Additional Qualifications
1.Must be able to satisfy the post offer requirements (i.e. Wisconsin Caregiver Background and medical screenings)
2.Some placements may have required use of a personal or company vehicle to transport consumer as needed. All roles requiring driving require a valid driver license and insurance
