Position Summary:

Performs a variety of support functions to assist in the aide or caring for residents.

Essential Functions:

Regular attendance and punctuality are essential to provide quality care
Must be in good physical condition as the position requires the ability to stand on one's feet throughout the day, as well as, push, pull, move and/or lift repetitively a minimum of 10 pounds up to greater than 50 pounds.
Must be able to complete performance requirements independently using good judgment as well as have the ability to harmoniously work with others.
Must be able to read, write and understand the English language.

Position Responsibilities:

Must follow BrookHaven's Policy and Procedures
Must meet job related competencies
Pass ice, water pitchers, linens, towels and washcloths
Mark and pass supplies, make beds and help serve plates and drinks to residents at meal times
May assist in feeding of residents if qualified per state guidelines
Set/clean off tables in dining area, clean dishes, put dishes away
Help pass snacks to residents
Transport residents to and from activities as well as conduct one-on-one activities with residents
Assist in various cleaning activities including: emptying linen/trash bins, cleaning wheelchairs, or other housekeeping duties
Perform other non-resident care duties as assigned

Knowledge, Skills and Abilities:

Ability to read, write, make simple calculations and follow oral or written instructions.
A minimum of a grammar school education is essential.
A high school diploma is preferred.

VIEW JOBS Full Time night shiftPart-time night shiftPosition Summary:Supervise nursing staff in accordance with facility policy and assume responsibility for the Assisted Living in the absence of the Director. Overall administration and supervision of both units including; ensuring quality care; training and supervision of professional and support personnel; disciplining and/or termination of unit personnel with guidance of human resources; administering personal hygienic, preventative, therapeutic and rehabilitation services to residents and assists in the coordination of nursing care provided by other disciplines.Essential Functions:<ul><li>Regular and punctual attendance</li><li>Manage nursing units which includes ensuring daily work assignments are completed, supervise the delivery of quality resident care</li><li>Communicate to Director changes in resident conditions, staffing issues or changes, concerns regarding personnel issues, transportation conflicts, etc.</li><li>Assist nurses in completion of tasks including medication administration, treatments, occurrences, managing staff, communication to families, etc. in order to effectively time manage and prevent overtime.</li><li>Round with physicians, share information necessary for care of residents, transcribe and implemented, ensure orders are signed.</li><li>Monitor faxes and communications to physicians; assure that orders are signed promptly according to policy.</li><li>Monitor completion of physician orders such as obtaining urine or stool samples.</li><li>Assure compliance with appropriate activities on Lakeside and give instructions to staff as needed.</li><li>Keeps units clean, hazard free and deals appropriately with emergencies.</li><li>Identifies and anticipates the needs of residents and staff and provide intervention as appropriate.</li><li>Maintains weekly charting on residents receiving skilled nursing care as required by ODH.</li><li>Orient new admissions to unit and insure that all systems are operation for the care and comfort of the resident.</li><li>Participates in the hiring, evaluation and termination process of personnel.</li><li>Train, supervises and provides ongoing in-service education to personnel, monitor staff attendance at monthly mandatory in-services.</li><li>Administer corrective action when policies are broken.</li><li>Assists nurse and unit personnel in interpreting and planning assignments and in executing difficult assignments.</li><li>Attends supervisory/managerial meetings, conferences and staff meetings.</li><li>Maintains confidentiality with regard to resident and employee records and other management information received.</li><li>Maintains and responsible for the proper use, care and treatment of unit equipment.</li></ul>Knowledge, Skills and Abilities:<ul><li>Graduate from an accredited school of practical nursing and currently licensed/registered with the Ohio State Board of Nursing to practice.</li><li>Must be or become familiar with organization, functions, policies and procedures and state regulations as they relate to nursing services and Assisted Living rules.</li><li>Must be committed to the standards of gerontological nursing and be licensed to administer medications according to state laws and have current CPR certification.</li></ul>

He/she shall be prepared to give resident care from prior experience, other training programs and/or in-service education programs provided by the facility. He/she shall work under the direction of the nurse.Essential Functions:<ul><li>Accurately measure and record information such as vitals, height, weight, blood sugars and other information as instructed.</li><li>Assist with bathing, dressing, grooming, nail care, oral hygiene, use of adaptive devices as needed, ambulating, positioning, turning and transferring residents into and out of bed, chairs, showers, wheelchairs, etc. keeping with resident safety needs.</li><li>Assist residents with bowel and bladder functions, and toileting programs; performs peri-care as needed.</li><li>Answer call lights promptly and ensure call lights are within reach of residents.</li><li>Participate in receiving nursing report, giving feedback to nurses on resident needs or changes in conditions, follow plan of care, explain procedures to residents before delivering care, honor resident preferences, use supplies economically and report resident supply needs timely to charge nurse for re-ordering. Determine resident needs and informs appropriate staff if unable to meet the need.</li><li>Ensure residents present a neat and clean appearance. Ensure resident clothing is clean and appropriate. Change bed linens as scheduled, using proper linen handling techniques in making and/or changing beds (occupied and unoccupied) and following infection control protocols. Collect soiled laundry/linen, wash, dry, fold and return to resident room timely. Complete laundry charges accurately.</li><li>Assist residents in preparation for and transporting to appointments, activities, social programs and dining areas for meals. Lead activities as assigned. Deliver mail and packages to residents.</li><li>Serve meals and snacks, striving for an excellent dining experience for the residents. Ensure supplements are given to residents as ordered by physician. Encourage residents to drink water. Set dining room tables for meals. Ensure residents are served meal consistency as ordered, report any problems with meal consumption, likes or dislikes and special preferences with the dietician and/or charge nurse.</li><li>Clean the dining room (tables, chairs, floors, etc.) and bus the tables, put away supplies and remove trash. Ensure kitchen is stocked with needed supplies for the residents, label and date food stored in the kitchen per policy, follow cleaning schedule, perform a variety of general cleaning activities including dusting, sweeping, vacuuming, mopping. Collect and dispose of garbage.</li><li>All other reasonable duties, as assigned.</li></ul>Knowledge, Skills and Abilities:<ul><li>High school diploma or GED preferred must be able to read, write and communicate in English.</li><li>Must have the ability to read labels and follow instructions relative to safe use of handling cleaning materials; must have the ability to operate a variety of equipment.</li><li>Previous experience in working with the elderly is preferred, but not required.</li><li>Basic first aid training is necessary within the first two weeks of employment.</li><li>Resident Aides must learn and be familiar with Residents' Rights.</li></ul>

He/she shall be prepared to give resident care from prior experience, other training programs, and/or in-service education programs provided by the Care Center. He/she shall work under the direction of the Registered Nurse or the Licensed Practical Nurse in providing safety measures and comfort to the resident.Essential Functions:<ul><li>Must have a high degree of integrity and refrain from ever discussing with an unauthorized person any information concerning the residents or the Center which he/she learns in the course of his/her work.</li><li>Must have the ability to work with others and to accept constructive criticism.</li><li>Must have a genuine interest and desire to help others and the ability to relate to residents with warmth and understanding.</li><li>Must appear clean and neat in uniform. The color and style of uniform to be determined by Administrator. Hose of appropriate color, white clinical shoes, and white undergarments are also part of uniform. Clogs and sandals are not acceptable. Hair should be short or up when working with residents. Good personal hygiene is required.</li><li>Perform such duties as are required in the physical care of the residents, as directed by the Charge Nurse.</li><li>All other duties as assigned.</li></ul>Knowledge, Skills and Abilities:<ul><li>Must be 18 years of age or a high school graduate for full-time employment.
