Well established Hospitalist program since 2006, currently includes 19 physician & 3 ARNP team
Care team support that includes 5 dedicated hospitalist RNs for care coordination and rounding efficiency + 1 CDE nurse
Excellent multi-specialty support
Fully integrated system with clinics and hospitals on EPIC EMR to improve care coordination
Full-time position
Hospitalist APP schedule is Monday
Responsibilities
Hospitalist Joint APP collaborates with orthopedic surgery, anesthesiology, hospitalist, nursing, therapy and hospital leadership to provide well-coordinated, high-quality, cross-continuum patient care to patients undergoing total joint replacement surgery at St. Luke's Cedar Rapids
Subject matter expert in total joint management and perioperative medicine
Review total joint list every week to identify same day discharge vs. overnight admission in collaboration with surgical services & joint academy
On day of surgery, hospitalist joint APP will be placed on patient treatment team and conducts physical assessments of post-operative joint patients including day of surgery discharges
Manages medical conditions of the post-operative total joint patients to the level of licensure and experience. Seeks consultation with Hospitalist physician or other services when appropriate
Evaluates, interprets and coordinates information to make decisions and recommendations regarding patient care
Patient-focused, friendly and willing to collaborate in a team environment
Excellent communication (written and verbal), analytical and problem-solving skills
Qualifications
Nurse Practitioner or Physician Assistant
Board Certified
Must be experienced with Ortho (total joints)
Inpatient experience as a nurse practitioner or registered nurse
Acute Care Nurse Practitioner preferred
Hold an unrestricted medical license in the state of Iowa
Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice
Eligible to work in the United States
