Cardinal Health manages the pharmacy at Dameron Hospital and we are currently recruiting for a full time Pharmacist.

Our Pharmacists provide pharmaceutical care services to the patients of the hospital including, but not limited to oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy as well as providing drug information, patient information and oversight of information and dispensing systems. The successful candidate will work with the team to ensure compliance with Federal, State and local laws and regulations as well as proper storage and documentation of pharmaceuticals.

Accountabilities:

  • Interprets medical orders (verbal and written) and transcribes to computerized patient medication profiles accurately. Maintains accurate, complete patient medication profiles.

  • Compounds and dispenses pharmaceuticals, including sterile, chemotherapy, and parenteral nutrition products accurately.

  • Issues controlled substances to patient care areas and maintains records as required by law.

  • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.

  • Assists with pharmacokinetics consult service and drug dosing per organizational protocol.

  • Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed.

  • Detects and reports suspected adverse drug reactions accurately and in a timely manner.

  • Sustains the formulary by minimizing nonformulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.

  • Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff, as needed.

  • Documents all clinical activities and interventions accurately and completely.

  • Participates in the quality improvement and medication use review activities of the department. Collects data; conducts quality monitors and inspections; and maintains logs, records, and other documentation as assigned.

  • Participates in the development and presentation of orientation, education, and training programs to the pharmacy, medical, nursing, and other staffs.

Qualifications

  • Must be a "good standing" licensed Pharmacist in California or willing to obtain

  • In-patient hospital pharmacy experience is strongly preferred

  • Flexibility to work various shifts

  • Ability to hear, write, and speak clearly in order to communicate with customers and health care professionals

  • Strong customer service skills.

  • Manual dexterity required for occasional reach, lifting and holding of small objects.

  • Computer savvy to learn and perform successful operation of multiple pharmacy information systems.

Additional considerations:

  • ACLS certified preferred

  • Previous Hospital Medication Safety program experience preferred

  • Previous hospital inpatient operations/workflow experience preferred

  • Previous Pyxsis best of practice experience preferred

  • Previous hospital 797/800 experience preferred

Cardinal Health manages the pharmacy at Dameron Hospital in Stockton, CA and we are currently recruiting for a full time Pharmacy Technician. Our Pharmacy Technicians demonstrate working knowledge of the policies and procedures regarding procurement, storage, preparation and delivery of pharmaceuticals in hospitals and health system settings. The successful candidate will process physician orders for review by a pharmacist and pick and prepare medications or supplies under pharmacist review. They will also restock supplies and medications, deliver supplies and medications, procure supplies and medications following standard purchasing guidelines and provide feedback on the stock level and on inventory control and delivery processes. Our Pharmacy Technicians are also skilled in own work area within an analytical/scientific method or operational process area. They will also expand skills and techniques in one skill area/process/method and maintain appropriate licenses/training/certifications, as needed. The successful candidate will apply knowledge and skills to complete assigned tasks within own area and work within established SOPs and/or scientific methods and adheres to quality guidelines while working with a moderate degree of supervision. Accountabilities: * Assists the Pharmacist in filling prescription orders, as permitted by the California Board of Pharmacy * IV Compounding * Handle doctor calls, faxes and medication deliveries * Organizes medications for pharmacist to dispense by profiling prescriptions; preparing labels and calculating quantities * Maintains a safe and clean pharmacy by complying with procedures, rules, and regulations * Takes inventory, places orders, checks in drugs and supplies, stocks shelves and removes out-of-date items from the inventory * Communicates with all customers (patients, clinics, care-givers, physicians) in a pleasant and professional manner * Other duties as assigned Qualifications * High school degree or equivalent * California Pharmacy Technician certification required * National pharmacy technician certification (PTCB) preferred * IV Certification a plus * In-patient hospital pharmacy experience highly preferred * Flexibility in working schedule * Ability to work well in a team * Strong verbal and written communication skills required * Strong customer service skills required * Comfortable performing repetitive motions/tasks is required * Ability to manage several tasks at the same time; Ability to focus on tasks ; Ability to evaluate operating conditions ; Ability to exercise sound judgment * Demonstrated ability to use technology such as computers, smart phones and tablets Cardinal Health is an Equal Opportunity/Affirmative Action employer.
