Providence at Home is a growing division of Providence Life Services, which provides a continuum of healthcare, focused on senior living. Providence Hospice is seeking a Hospice Social Worker, Part Time in Tinley Park, IL. We are looking for a compassionate Social Worker to join our established, experienced hospice team. We provide exceptional patient centered care to patients and families facing end of life.

Schedule for position: Must be able to work (3) days per week 8:00AM-4:30PM.

Summary of essential job functions:

• Evaluate patient, family needs and support them through the end stages of life and to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

• Ordering Durable Medical Equipment and planning discharges from hospitals and/or nursing homes.

• Visiting patients and families on a weekly, bi-monthly, or monthly basis to provide ongoing counseling and support as patients are transitioning and dealing with end of life issues.

Requirements

• Masters degree in Social Work.

• Licensed Social Worker in the State of Illinois.

• Minimum of 1 year Hospice experience preferred.
