Job Description

Phoenix Home Care & Hospice is currently seeking a Social Worker for our Hospice Department in the Topeka, KS area.

POSITION DETAILS

  • Case Info: Social Worker for Hospice patients from the Topeka area.

  • Our Hospice Positions include: meeting the physical, spiritual and emotional needs of our clients and their families. Our patients are facing end-of-life healthcare needs and we are seeking Hospice Team Members who can provide the needed nursing care with dignity, compassion & a warm heart.

DUTIES AND RESPONSIBILITIES

  • Conduct a psycho-social assessment as part of the comprehensive assessment within 5 days of admission to the hospice program. This includes emotional, social, financial and environmental resources and to identify appropriate psycho-social problems, interventions, and goals for the patient's plan of care.

  • Participate in IDG meetings and the development of the patient's plan of care.

  • Observe, record and report the changes on the patient's emotional and social factors that are being affected by the patient's illness and his/her need for care and his/her response to treatment.

  • Maintain and submit written clinical records as deemed by the Agency, including the initial evaluation, the care plan and daily notes.

  • Evaluate the patients and family's response to, and effectiveness of, the medical social work intervention.

  • Confirm, on a weekly basis, the scheduling of visits with the patients' Case Managers to facilitate coordination of other staff visits.

This position is responsible for the assessment of the patient/family#s psychosocial needs. Provides crisis intervention, grief counseling, and other psychosocial supports when indicated.# Identifies gaps in services, care and equipment and evaluates resources available. Collaborates with the care team to prioritize needs and makes recommendations for coordination of care. Works in tandem with the RN Case Manager to meet the needs of the patient and develop a successful transition/discharge plan. This position requires dialogue with individuals regarding difficult topics and requires effective communication skills. # Essential Duties and Responsibilities:include the following. Other duties may be assigned. Triage and assess patients/families in the ED for psychosocial needs including conflict/isolation, treatment decisions, quality-of-life issues and transitions in care, advance directives, domestic violence, abuse, neglect, coping/communication, and functional changes. Explore and evaluate solutions for problems that are illness related as well as concrete needs such as housing, food, financial assistance, education, employment, language and cultural barriers when indicated. Facilitates referrals to appropriate resources both internally and externally. Educates patients about resources and options for levels of care as needed. Initiates contact with reporting agencies as needed or directed e.g.DCF, APS, KPRS, and KDADS. Communicates in a clear and concise manner with patients/family/caregivers regarding patient status and progress. Adjusts communication style responsive to receiver. Develops a discharge/transition plan in conjunction with the ED care team to achieve the best outcome. Collaborates with all members of the health care team to effect a smooth transition to the next level of care. Is a member of the ED Behavioral Response Team. Works collaboratively with the CM, ED, UR staff, and SUD coordinators. Cultivates and maintains positive relationships with community agencies. Collaborates effectively to reduce avoidable visits to the ED and/or hospital admissions Maintains confidentiality and complies with professional ethics according to professional, departmental, and organizational standards. Demonstrates excellent communication skills working daily with health care providers, internal departments, external agencies and community resources. Maintains a working knowledge of statutes related to EMTALA, Mental Health, DPOA, legal guardianships, and abuse and neglect. Utilizes clinical documentation, statistical reporting and evaluation to improve services, assure quality, and develop programs. Maintains a professional level of productivity and coordinates and prioritizes patient related activities in an efficient and effective manner. Demonstrates initiative and provides leadership in the face of changes and/or development of new programs or services. Supports and supervises Social Work practicum students as needed. # Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
