The purpose of this position is to provide services to meet the social and/or emotional needs that affect the clients ability to achieve their goals; participate in the development of patent/client comprehensive care plans; develop policies to provide social services to clients in compliance with federal, state and local regulations. This position is supervised as designated on the agency organizational chart.

Employment Status: Full-Time

Typical Hours of Employment: Monday - Friday 8a-5p with on-call rotation

Required: Masters degree in Social Work.

Current unrestricted State of Kansas license to practice as a Social Worker. Three to five years of professional social work experience in health care or mental health preferred, 1 year experience working with older adults. Must have knowledge and skills with regard to health care coordination, pain/symptom management, crisis intervention and counseling. Must possess valid Kansas Drivers License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organizations insurability standard as defined by Midland policy.

Physical Demands

Must have hearing acuity sufficient to hear verbal instructions and communication from clients, respond to emergency alarms, and answer the telephone. Must be able to concentrate on moderate details with frequent interruptions, attend to specific functions for 10-25 minutes, remember multiple tasks and assignments over and 8-12 hour period, and understand and relate specific ideas and concepts.

Must be able to deal effectively with stress, clients and their families, multiple tasks, noise, interruptions, and emotional loss. Must demonstrate the ability to work cooperatively with other members of the interdisciplinary team while maintaining a pleasant and professional demeanor. Must be able to travel to and access client homes where conditions may be adverse due to weather, environment, or clients lifestyles.

Work Environment

Individual must wear business attire and spends virtually all of the time inside. Work location is split between on campus appointments and community appointments including private homes and health care/residential facilities.

This position is responsible for the assessment of the patient/family#s psychosocial needs. Provides crisis intervention, grief counseling, and other psychosocial supports when indicated.# Identifies gaps in services, care and equipment and evaluates resources available. Collaborates with the care team to prioritize needs and makes recommendations for coordination of care. Works in tandem with the RN Case Manager to meet the needs of the patient and develop a successful transition/discharge plan. This position requires dialogue with individuals regarding difficult topics and requires effective communication skills. # Essential Duties and Responsibilities: include the following. Other duties may be assigned. Triage and assess patients/families in the ED for psychosocial needs including conflict/isolation, treatment decisions, quality-of-life issues and transitions in care, advance directives, domestic violence, abuse, neglect, coping/communication, and functional changes. Explore and evaluate solutions for problems that are illness related as well as concrete needs such as housing, food, financial assistance, education, employment, language and cultural barriers when indicated. Facilitates referrals to appropriate resources both internally and externally. Educates patients about resources and options for levels of care as needed. Initiates contact with reporting agencies as needed or directed e.g.DCF, APS, KPRS, and KDADS. Communicates in a clear and concise manner with patients/family/caregivers regarding patient status and progress. Adjusts communication style responsive to receiver. Develops a discharge/transition plan in conjunction with the ED care team to achieve the best outcome. Collaborates with all members of the health care team to effect a smooth transition to the next level of care. Is a member of the ED Behavioral Response Team. Works collaboratively with the CM, ED, UR staff, and SUD coordinators. Cultivates and maintains positive relationships with community agencies. Collaborates effectively to reduce avoidable visits to the ED and/or hospital admissions Maintains confidentiality and complies with professional ethics according to professional, departmental, and organizational standards. Demonstrates excellent communication skills working daily with health care providers, internal departments, external agencies and community resources. Maintains a working knowledge of statutes related to EMTALA, Mental Health, DPOA, legal guardianships, and abuse and neglect. Utilizes clinical documentation, statistical reporting and evaluation to improve services, assure quality, and develop programs. Maintains a professional level of productivity and coordinates and prioritizes patient related activities in an efficient and effective manner. Demonstrates initiative and provides leadership in the face of changes and/or development of new programs or services. Supports and supervises Social Work practicum students as needed. # Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
