  • Minimum of 2 years as an RN required

  • Current & active nursing license and current CPR certification required

  • Availability to work Full Time Mon-Fri

  • Make patient visits at their place of residence or facility

  • Call will be required (nights, holidays and weekends)

  • Knowledge of EMR is required

  • Previous Home Health or Hospice experience needed

  • Compassionate RN with great time management skills

If you are located in the greater New Orleans, LA, you will have the flexibility to telecommute* as you take on some tough challenges. This role involves approximately 75% travel and you'll need to be comfortable dealing with a full range of in-home and facility-based environments. Most of the time you'll work autonomously, so the ability to stay focused and motivated is essential. Primary Responsibilities: * Visit members in their homes and/or other settings, including community centers, hospitals or providers' offices * Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction * Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels * Assist members with the transition from a care facility back to their home You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: * Current, unrestricted RN license in the state of Louisiana * 2 years clinical experience in a hospital, acute care, home health/hospice, direct care or case management * Intermediate computer/typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications * Must be able to travel 75% to members home, hospitals and facilities Preferred Qualifications: * Case management experience including Certification in Case Management * Home care/field based case management * Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs * Experience working with the Medicaid population * Experience in Home & Community based or Long Term Care services delivery Careers at UnitedHealthcare Community & State.
