Hospice Patient Care Manager

Hunterdon#Hospice - an affiliate of Hunterdon Health - is seeking highly qualified RN candidates to staff its growing program. The successful candidate will assist with admissions, on-call duties, routine visits, and case management for hospice patients residing throughout Hunterdon County # surrounding areas. # Previous hospice or home health nursing experience preferred. • Current New Jersey RN License. If associates degree or diploma must be enrolled in BSN program and completed within 5 years of hire date.
