Provides diagnosis, treatment, consultation, and follow-up (depending on the state licensure) under the supervision of 1 or more physicians in the practice. Provides age and specialty appropriate medical care at the level of training achieved. Should at a minimum assess: disease-specific changes, decline or change in function, nutrition and/or cognition, inadequately controlled pain or other distressing symptoms, falls, infections or similar events, the impact of the disease burden on the patients condition, caregiver and/or familys quality of life.
REQUIRED EDUCATION / EXPERIENCE
Graduate from an accredited school of nursing and certified as a Nurse Practitioner. Masters of Science in Nursing. Specialty in Family, Adult, Geriatric or Acute Care preferred. A minimum of two years Nurse Practitioner experience is required. Licensed and credentialed.
1.* Abides by and demonstrates the Company Mission Vision Values through both behavior and job performance on a day-to-day basis.
2.* Gathers clinical information to support the diagnosis and prognosis and provides these findings from the face to face encounter to the physician who is considering recertifying the patient.
3.* Attests to a face to face encounter being made and includes in the attestation the beneficiarys name, the date of the encounter, the certification period dates, and that the clinical findings of that encounter have been provided to the certifying physician for use in determining continued eligibility for hospice care.
4.* Communicates clinical findings of the face to face encounter to the Interdisciplinary Group for use in coordinating patient care.
5.* Provides care to patient when a need is identified during a face to face encounter.
6.* Documents the clinical finding and care provided in the patients medical record.
7.* Establishes and maintains effective working relationships with those contacted in the course of work.
8.* Practices in accord with the laws and regulations of the state of practice, including collaboration with or supervision by a physician.
9.* Practices within the scope of practice for which he or she was trained and is certified.
10.* Adheres to and participates in Companys mandatory HIPAA privacy program/practices and Business Ethics and Compliance programs/practices.
11.* Reviews and adheres to all Company policies and procedures and the Employee Handbook.
12.* Participates in special projects and performs other duties as assigned.
