Provides diagnosis, treatment, consultation, and follow-up (depending on the state licensure) under the supervision of 1 or more physicians in the practice. Provides age and specialty appropriate medical care at the level of training achieved. Should at a minimum assess: disease-specific changes, decline or change in function, nutrition and/or cognition, inadequately controlled pain or other distressing symptoms, falls, infections or similar events, the impact of the disease burden on the patients condition, caregiver and/or familys quality of life.

Graduate from an accredited school of nursing and certified as a Nurse Practitioner. Masters of Science in Nursing. Specialty in Family, Adult, Geriatric or Acute Care preferred. A minimum of two years Nurse Practitioner experience is required. Licensed and credentialed.

1.* Abides by and demonstrates the Company Mission Vision Values through both behavior and job performance on a day-to-day basis.
2.* Gathers clinical information to support the diagnosis and prognosis and provides these findings from the face to face encounter to the physician who is considering recertifying the patient.
3.* Attests to a face to face encounter being made and includes in the attestation the beneficiarys name, the date of the encounter, the certification period dates, and that the clinical findings of that encounter have been provided to the certifying physician for use in determining continued eligibility for hospice care.
4.* Communicates clinical findings of the face to face encounter to the Interdisciplinary Group for use in coordinating patient care.
5.* Provides care to patient when a need is identified during a face to face encounter.
6.* Documents the clinical finding and care provided in the patients medical record.
7.* Establishes and maintains effective working relationships with those contacted in the course of work.
8.* Practices in accord with the laws and regulations of the state of practice, including collaboration with or supervision by a physician.
9.* Practices within the scope of practice for which he or she was trained and is certified.
10.* Adheres to and participates in Companys mandatory HIPAA privacy program/practices and Business Ethics and Compliance programs/practices.
11.* Reviews and adheres to all Company policies and procedures and the Employee Handbook.
12.* Participates in special projects and performs other duties as assigned.

VIEW JOBS 11/7/2018 12:00:00 AM 2019-02-05T00:00 The family nurse practitioner (Urgent Care) is a licensed practitioner, who under the general guidance of the delegating physician, is responsible for the provision and plan of care for HHC Med Stop patients. Duties include obtaining the patient history, performing a physical examination, diagnostic testing, ordering medications and treatment interventions to conclude a diagnosis, and treatment plan for the patient illness or injury. The practitioner will provide education regarding the diagnosis, interventions and plan of care given to patient and family members inclusive of applicable medical options, and the care continuum process. The practitioner may prescribe medications and treatment plans. The practitioner assesses patient conditions and must be able to identify a patient presenting or progressing to an unstable status, upon at which time interventions and the plan of care changes to an emergency state in which EMS is activated and lifesaving interventions are initiated utilizing the available resources at the Med Stop. The practitioner is responsible for clear, concise, effective communication with other medical professionals within and outside of Houston Healthcare and must facilitate confidentiality, and privacy. The practitioner will also represent HHC with high degree of visibility within the community and thus must possess positive leadership skills, participate in collaborative efforts, and demonstrate a strong sense of community in alliance with the mission, vision and values of HHC. Graduate of an accredited school of nursing. Graduate of an accredited Nurse Practitioner program. Minimum three (3) years RN experience working as a clinical nurse in an Intensive Care Unit or Emergency Department. Current RN licensure by the Georgia State Board of Nursing. Current AHA-BLS certification. Current ACLS and PALS or obtained within 6 months of hire. Current Advanced Practice Registered Nurse (APRN) or Advanced Practice Nurse Practitioner (AANP) Licensure. Current Nurse Practitioner license. High level, clinical decision making, ability to establish and maintain effective, respectful relationships with patients, families, hospital and clinic staff, faculty, and physicians. Strong computer skills, including Word, Excel and PowerPoint. Successful candidates will be subject to criminal background and drug screens.
