JOB OVERVIEW:

A licensed and credentialed Nurse Practitioner (NP) provides diagnosis, treatment, consultation and follow-up under the supervision of 1 or more physicians. The NP provides age- and specialty-appropriate medical care at the level of training achieved, and is an extension of a physician with the ability to provide full hospice across all patient types.

My commitment will include:

  • Effectively identifying, evaluating and addressing hospice and chronic disease management issues of the population in the practice while administering quality patient care.

  • Maintaining a full case load as directed by the physician.

  • Communicating significant findings, problems and changes in condition or environment to the Clinical Supervisor, physician, facility and/or other personnel involved with patient care.

  • Diagnosing and treating acute, chronic and long-term healthcare issues.

  • Working in an independent and interdependent relationship with members of the medical staff, allowing for consultation, collaboration or referral.

  • Administering therapeutic procedures; prescribing therapy or medication with physician approval.

  • Effectively conducting a face-to-face assessment within time frames set forth by CMS to determine hospice eligibility.

  • Maintaining legible, accurate and confidential medical records; documenting all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals and consultations consistent with state regulatory standards.

  • Educating patients and/or families about hospice care, medical issues and use of prescribed medical treatments and/or medications.

Qualifications:

  • Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.

  • Licensure: Current State License as a Registered Nurse. Certification as a Nurse Practitioner.

  • Experience: 2 years' experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred.

  • Skills:
    Nursing skills as defined by accepted nursing standards. Demonstrates ability to assess and respond to the needs of patients, families and caregivers in varied settings and to function effectively in the home or nursing facility environment. Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles. Ability to handle multiple priorities, documentation requirements and deadlines.

The Hospice Aide provides personal health care, and related services and additional tasks as assigned to the client in the home or facility. My commitment will include: * Providing superior quality aide care whenever that care may needed, including a weekend rotation. * Providing a safe and hygienic environment for each individual patient I care for. * Working cooperatively with patient and family. Sharing observations with my supervisor so we can deliver high quality care as a team. * Working an on-call weekend rotation to meet the needs of patients 24/7. * Promoting positive, supportive, respectful communication to patient, family and other employees. * Provides direct compassionate patient care to patients under the direction of the Registered Nurse to ensure the proper plan of care is carried out. * Providing necessary skills to appropriately report changes and document pertinent information and care rendered to the patient to ensure continuity of care occurs for each patient I care for. * Providing appropriate skills necessary to assist the patient with proper nutrition and adequate fluid intake. Qualifications: * Education: Must provide satisfactory evidence of successful completion of supervised practical training totaling at least 75 hours, with at least 16 hours devoted to supervise practical training. * Licensure: Must maintain certification as a Licensed Nursing Assistant (C.N.A. or STNA) in applicable state. * Experience: At least 1 year experience in the last 24 months as a Hospice Aide, a Home Health Aide or a Nursing Assistant in a hospital, long term care facility or home health agency. * Skills: Compassionate and caring. Must be able to read and write and carry out directions in English. Demonstrates a sympathetic attitude toward the care of the sick and desires to provide care in the home. Ability to pass competency evaluation prior to performing care. Must be self-motivated. * Transportation: Reliable transportation.
