A licensed and credentialed Nurse Practitioner (NP) provides diagnosis, treatment, consultation and follow-up under the supervision of 1 or more physicians. The NP provides age- and specialty-appropriate medical care at the level of training achieved, and is an extension of a physician with the ability to provide full hospice across all patient types.
My commitment will include:
Effectively identifying, evaluating and addressing hospice and chronic disease management issues of the population in the practice while administering quality patient care.
Maintaining a full case load as directed by the physician.
Communicating significant findings, problems and changes in condition or environment to the Clinical Supervisor, physician, facility and/or other personnel involved with patient care.
Diagnosing and treating acute, chronic and long-term healthcare issues.
Working in an independent and interdependent relationship with members of the medical staff, allowing for consultation, collaboration or referral.
Administering therapeutic procedures; prescribing therapy or medication with physician approval.
Effectively conducting a face-to-face assessment within time frames set forth by CMS to determine hospice eligibility.
Maintaining legible, accurate and confidential medical records; documenting all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals and consultations consistent with state regulatory standards.
Educating patients and/or families about hospice care, medical issues and use of prescribed medical treatments and/or medications.
Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
Licensure: Current State License as a Registered Nurse. Certification as a Nurse Practitioner.
Experience: 2 years' experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred.
Nursing skills as defined by accepted nursing standards. Demonstrates ability to assess and respond to the needs of patients, families and caregivers in varied settings and to function effectively in the home or nursing facility environment. Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles. Ability to handle multiple priorities, documentation requirements and deadlines.
