We are seeking a Hospice Case Manager (RN) for a full-time opportunity with our team in San Diego, California!

The Hospice Case Manager will provide care to the terminally ill Hospice patient as needed as well as assistance and understanding to the family in the home care situation and in time of bereavement. The Hospice Case Manager is a member of the Hospice Team in providing Hospice care and we hope that they will have experience in nursing, with emphasis on Hospice education.

Compensation Package:

• $53 to $70 Per Hour (Based on Experience)

Requirements:

• Registered Nurse Licensed in the State of California
• 1+ Years of RN Experience- Ideally in the Hospice Setting
• Minimum Associate's Degree in Nursing
• Utilizing Kinnser- Training Offered
• Spanish Speaking Individual Preferred but NOT Required
• Maintains a current CPR and BLS certification
• Current and Active Driver's License
• Must Have a Compassionate and Can-Do Attitude

Responsibilities:

• Monday through Friday 8 AM to 5 PM
• Provide Home Hospice Visits (Approximately 4 Per Day) in either North or South County San Diego
• North County: La Jolla, Oceanside, Carlsbad, and Encinitas Area(s)
• South County: Chula Vista, East County, National City, and San Diego Proper Area(s) 
• Regularly Assesses and Reassesses the Nursing Needs of the Hospice Patients
• Provides Hospice nursing services, Treatments, and Preventative Procedures
• Observe Signs and Symptoms and Report to the Physician and IDG Members any Unexpected Changes in the Patient’s Physical or Emotional Condition
• Teaches, Supervises, and Counsels the Hospice patient and Family Members about Providing Care for the Patient
• Develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care
