Assists in the planning, implementation, documentation and coordination of patient care services to Hospice patients and their families. Calvary is the countrys only hospital dedicated to providing hospice and palliative care to adult patients with advanced cancer and other life-limiting illness. CalvaryCare is available at four locations in the Bronx, Brooklyn, Manhattan and Queens and throughout the greater New York area where people reside.

Requirements

  1. Current NYS Registered Nurse license.
  2. Baccalaureate Degree in Nursing from an accredited school is preferred.
  3. Two (2) years experience in nursing which includes any combination of medical-surgical, oncological, palliative and community home health/hospice.
  4. At least one (1) year of community hospice or home health experience is preferred.
  5. Hospice experience preferred.
  6. NYS drivers license.
  7. Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.

The Bereavement Counselor is responsible for assisting the Director of Bereavement in training volunteers/interns, participating in educational programs and facilitating support groups for bereaved children, teens, and adults.</p><ul> <li>Plans and facilitates Bereavement Support Groups for children, teens, and adults.</li> <li>Conducts intake interviews to appropriately place individuals in support groups.</li> <li>Counsels and/or refers individuals to outside clinicians as appropriate.</li> <li>Maintains client records for individuals and attends bereavement programs.</li> <li>Conducts outreach to strengthen existing programs by building relationships with new (and existing) referral sources including hospitals, schools, and clinicians</li> <li>Collaborates with Director of Bereavement and Bereavement Teams to coordinate Camp Campus events for bereaved children during the summer</li> <li>Works within legal, ethical practice standards to his/her position as well as Calvary’s Code of Conduct.
