TITLE: Hospice Medical Director (Contractor)

OFFICE LOCATION: South San Francisco

PATIENT TERRITORY: San Francisco

SCHEDULE: As Needed

SHIFT: 8:30am-5pm, and on call 5pm - 8 am

POSITION SUMMARY:

The Hospice Medical Director is a Board Certified licensed physician in the State of California and Hospice and Palliative Care Certified who assumes overall responsibility for the medical component of the patient care program and for the supervision, scheduling and oversight of the Hospice Physician team and Nurse Practitioners, assesses appropriateness of admissions, certifies terminal prognosis and participates in establishing, reviewing and revising plans of care for Hospice patients; participates in development of in service education and quality assurance programs; acts as a medical resource/liaison to physicians and health care providers; and acts as a resource for and member of the Interdisciplinary Group (IDG). In conjunction with these administrative duties, the Hospice Medical Director serves as the Hospice Physician for the Sunnyvale Hospice Clinical Teams (IDG) and provides oversight for the clinical component of the Hospice patients' care including visiting the patient and families for evaluation and assessment when appropriate.

AREAS OF RESPONSIBILITY:

  • Responsible for the administrative management and direction of the Hospice Medical Director meetings.

  • Coordinates with the Pharmacy Services.

  • Coordinates with the Director of Quality Improvement, Education and Compliance to assure that policies and procedures are in place for areas within the scope of medical practice.

  • Assumes the overall responsibility for the medical component of the Hospice's patient care program.

  • Responsible for the hiring and orienting Hospice Physicians.

  • Supervises and evaluates performance of the Hospice Physician staff.

  • Assures availability of physician services to Hospice patient care staff for emergency situations in the event that the patient's attending physician is unavailable; coordinates the schedules of the Hospice Physicians to assure availability of Hospice Physician at all times (24 hours a day, seven days a week.)

  • Provides oversight of physician services and, in addition to palliation and management of the terminal illness and related conditions, meets the general needs of the patients to the extent that these needs are not met by the attending physician. This includes visits to patients for the purpose of evaluation and assessment of status.

  • For the Hospice patient, participates in the assessment of admissions for appropriateness and level of care, in establishing the Interdisciplinary Group Initial Plan of Care and in subsequent reviews and revisions per agency policy and procedure.

  • Certifies that the Hospice patient is considered terminally ill with a life expectancy of six months or less, if the terminal illness runs its normal course.

  • Acts as the Agency's Laboratory Director as defined by the Clinical Laboratory Improvement Act (CLIA).Provides supervision of Associate Medical Directors/IDG physicians.

  • Arranges for coverage of duties, if unable to perform these duties, assuring that covering physicians comply with all requirements of this job description.

  • Establishes, with the IDG, the policies governing the day-to-day provision of hospice care and services.

  • Attends team meetings as the physician member of the IDG, acting as a medical resource.

  • Acts as a medical liaison and provides professional education to physicians, health care providers, and other organizations or individuals with which the Hospice may interact.

  • Assists in development of in-service education and quality assurance programs.

  • Performs duties of Hospice Medical Director without regard for race, religion, national origin, age, sex, sexual preference or ability to pay.

  • Complies with all agency compliance requirements, such as maintaining a current PPD.

  • Performs other duties as assigned consistent with sills and training and the mission and goals of Pathways.

QUALIFICATIONS:

  • Board Certified with current unlimited license to practice medicine in the State of California.

  • Hospice and Palliative Care Certification.

  • Current controlled substance Registration certificate issues by the U. S. Department of Justice Drug Enforcement Administration.

  • Current California Driver's license and automobile insurance.

  • Current CPR certification.

  • Demonstrated basic computer proficiency.
