Licensed Practical Nurse Position Summary
Participate in directed care delivery including functional and focused nursing tasks as assigned by the RN
Provide ADL and other support as directed
Report observations and complete all required documentation in a timely manner
Function as part of a nursing team supervised by a Registered Nurse
Principal Accountabilities
Submit appropriate documentation and paperwork to facilities at the completion of patient visits
Submit all required documentation in a timely manner
Attend and participate in staff meetings, educational programs and community events as requested
Communicate identified needs and potential solutions to supervisor
Complete documentation at the bedside
Adhere to standards of nursing practice and maintain knowledge of current clinical practice for terminally ill patients
Participate in quality performance improvement projects and clinical record reviews as directed
Communicate effectively with RNCM, DCS and physicians (as applicable)
Provide nursing services as directed by the RN
Administer medications, treatments, etc. as directed
Gather clinical data and provide to RN via documentation and communication in order to support comprehensive assessment and Plan of Care management
Attend IDT meetings when able
Provide education and reinforcement of teaching related to:
Care of the patient
Disease process
Symptom management
Medications, including expected effect, side effects, and when to call hospice
Wound care
The dying process
Education and Experience
High school diploma or general education degree
Current nursing license for the specified state and minimum two years full-time experience as a Licensed Practical/Vocational Nurse; or equivalent combination of education and experience
Experience in skilled nursing, preferably in hospice, home health care, oncology or emergency care
Performance within scope of practice per state Nurse Practice Act is required
Strong written and verbal communication skills
Strong mathematical and problem-solving skills
Knowledge of Microsoft Office programs
Experience with EMR (electronic medical records) preferred
Skills and Abilities
Understanding and acceptance of hospice philosophy
High personal integrity including maintenance of confidential information and all patient records
Strong organizational and time management skills
Must be able to work a flexible schedule including nights and weekends
Must be able to travel
Ability to work with patients who are terminally ill and their family members
Ability to understand, read, write, and speak English
Physical demands include:
Ability to use hands to finger, handle or feel
Ability to talk and hear
Ability to stand, sit and reach with hands and arms
Ability to walk, climb, balance, stoop, kneel, crouch or crawl
Ability to taste and smell
Ability to lift and/or move more than 50 pounds
Specific vision abilities including close vision

Certifications, Licenses and Registrations
A valid driver's license and auto liability insurance
Licensed Practical Nurse in the state of employment
Current CPR certification.
