Weekday reserve (per-diem) Hospice Liaison is a Registered Nurse position to cover for time off and vacations as well as additional help on busy days. The Hospice Liaison will represent St. Luke's Hospice to build and enhance relationships with referral sources and facilitate the processing of hospice referrals. The Hospice Liaison will use clinical knowledge and empathetic communication skills to explain and educate about hospice services and evaluate patients for hospice eligibility. The Hospice Liaison will collaborate with other VNA departments to assure that the referral sources' needs are met and that patient's progress seamlessly along the continuum of care. He/she will work cooperatively with the Hospice team to identify, plan and pursue growth opportunities.
1.Facilitates processing of hospice referrals and continuity of patient care by acting as the Hospice representative at local health care hospitals, facilities and the community.
2.Communicates with Intake Department and Clinical Managers re: availability of resources and referral capacity.
3.Acts as a resource to hospital staff and the community regarding hospice.
4.Validates referral information. Facilitates development of the initial plan of care with Physicians, including evaluating for hospice eligibility and communicating within the VNA , and with other community resources and vendors to assure continuity and appropriateness of all services to meet the patient's needs.
5.Understands Third party coverage to assure appropriate financial coverage for services ordered. Communicates any financial responsibility to the patient and family as necessary.
6.Develops and in-depth understanding of the needs and preferences of each referral source and the associated opportunities for the VNA to satisfy their needs.
7.Plans and presents educational forums to hospital staff, medical community and community at large as requested.
8.Participates in department initiatives in marketing all VNA programs to the Network and community.
9.Maintain current knowledge of hospice regulations, industry trends and reimbursement issues.

PHYSICAL AND SENSORY REQUIREMENTS:

Medium Work Exertional activity, Occasionally lift and/or carry 50 lbs.

EDUCATION:

Graduate of State approved School of Nursing required. Bachelor's degree preferred.

TRAINING AND EXPERIENCE:
1.Minimum of 3 years of successful, generalized community health nursing, one year with liaison responsibilities in a similar setting preferred.
2.Must possess initiative, good problem-solving skills, and negotiating abilities.
3.Must have a general familiarity of Microsoft Office and Outlook software.
4.Hospice experience preferred.

Delivers professional nursing services to patient and families assigned at the Inpatient Hospice House, and demonstrates the ability to assess, plan, evaluate and implement nursing care with attention to the special needs of the agency. Works in accordance with the Team's Plan of Care and written agency programs and policies. JOB DUTIES AND RESPONSIBILITIES: Evaluates and does an assessment on all assigned patients in the inpatient facility. Considers psychosocial, environmental, spiritual and financial needs. Communicates with the patient's physician and/or hospice Medical Director, both orally and in writing as required by the patient's condition and agency policy. Cooperates with the Volunteer Coordinator and is available to hospice volunteers for counsel, supervision, advice and training when dealing with the patients and families. Assumes general personal responsibility to keep informed of current changes and trends affecting Hospice care and professional nursing functions. Determines eligibility for admission; obtains financial and statistical data; formulates initial care plan according to the patient's condition. Carries primary responsibility for providing comprehensive nursing care, health teaching, guidance and supervision to the individual and family while in inpatient facility. Supervises patient care given by Hospice Aides. Maintains and uses patient clinical records, reports and other written communications according to agency policy and directives. Clinical records are written daily. Records make clear the goals and plans with emphasis on the family oriented approach to patient care. Records reflect contact with physicians and other agencies needed. Participates in the development and operation of the agency by: Keeping administration informed on changing community needs and lack of community resources. Participating in the orientation and guidance of new staff. Interpreting agency service to families and community groups. Contributing to evaluation of service programs. Evaluating effectiveness of his/her own service and contributing to the evaluation of and continuing development of staff members. Participating in the development of inservice programs and group teaching for staff and other groups, as assigned. Acts as triage nurse in accordance with agency on-call policy. Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information. Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements. Demonstrates/models the Network's Service Excellence Standards of Performance in interactions with all customers (internal and external). Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety. Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices. Complies with Network and departmental policies regarding attendance and dress code. Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated. PHYSICAL AND SENSORY REQUIREMENTS: Sit up to 2 hours per day; 1 hour at a time. Stand for up to 10 hours per day; 8 hours at a time. Walk 6 hours per day; 10 minutes at a time. Consistently lift, carry and push objects up to 10 lbs. Occasionally lift, carry and push objects up to 75 lbs. Transports patients weighing up to 500 lbs. via wheelchair, bed and/or stretcher. Frequently stoop and bend. Frequently reach above should level. EDUCATION: Current RN license in the State of Pennsylvania. TRAINING AND EXPERIENCE: One year medical/surgical experience preferred. Individualized orientation program for all areas. Evidence of successful completion of BLS.
