We are looking for a RN Hospice Liaison (Admissions Coordinator) for St Joseph Health!
Location: Queen of the Valley Medical Center - Napa, CA
Schedule: Full time, 80 hours biweekly
Shift: 8 hour, Day
Positively promotes our hospice programs with physicians, case managers and social workers within the area hospitals and home care systems served by hospice. Promotes access to care and preferred provider status for the hospice programs. Serves as an internal and external resource to provide end-of-life care education and consultation services as appropriate.
Demonstrates ability to plan, develop and manage an area-wide medical community liaison program, which meets department goals.
Builds and maintains relationships with physicians, case managers and other hospital and home care staff to promote referrals and address needs, issues and concerns specific to hospice care.
Demonstrates that teamwork and communication are the primary keys in relationships. Creates a culture that fosters open, honest and respectful exchange. Promotes teamwork, mutual respect, participation and effective coordination of resources.
Promotes hospice services through an effective education and consultation program targeted for the healthcare community.
Education: Registered Nurse or Masters degree in Social Work
Experience: Minimum of 2 years experience in hospice environment.
License/Certification: Current Registered Nurse California license or LCSW.
Valid California driver's license.
