We are looking for a Home Health Aide for the Hospice department at St Joseph Health!
Location: Santa Rosa Memorial Hospice, Santa Rosa, CA
Schedule: Per Diem
Shift: 8 hour, Day
Works under the supervision of a registered nurse to provide personal care and support services to patient/family members.
Demonstrates knowledge and skills in the personal care needs of the dying patient.
Demonstrates knowledge and skills in providing for the nutritional care needs of the dying patient.
Demonstrates knowledge and skills in providing for the mobility needs of the dying patient.
Demonstrates knowledge and skills in providing for the skin care needs of the dying patient.
Demonstrates knowledge and skills in providing for bowel and bladder elimination of the dying patient.
Demonstrates knowledge of environmental safety in the home.
Demonstrates sound understanding of and commitment to the hospice philosophy of team.
Demonstrates understanding and role related to the following policies: Suicide, Elder and Child Abuse and substance abuse.
Demonstrates knowledge and skills in providing care for the patient with a Foley catheter.
Demonstrates knowledge and skills in providing care for the patient using oxygen.
Demonstrates sound understanding of care for the post mortem patient.
Demonstrates understanding of Standard Precautions and isolation techniques.
Demonstrates understanding of patient care in facilities, including SNFs, board and care and assisted living facilities.
Recognizes the need to establish rapport with patients and families.
Recognizes the need to maintain professional boundaries.
Demonstrates ability to identify changes in condition that warrant contacting RN, SW or Chaplain. Is able to discern priority of need in passing on such information and uses good judgment in choice of method used to relay the information.
Education: High school diploma preferred.
Experience: Minimum 1 year of experience as a Home Health Aide in the last 3 years, or equivalent.
License/Certification: Home Health Aide Certified by the State of California.
