Hospice Clinical Liaison

If you are a licensed RN with a hospice heart and a minimum of 2 years of healthcare experience, we would love to hear from you. Responsibilities: Facilitating timely intake and referral processes for patients Completing admissions processes, particularly in emergency cases Providing accurate and timely information about hospice services to inquiring individuals Advocating for patients and developing their care plans Building relationships with various healthcare providers and offering education on the benefits of hospice Requirements: Compassionate, kind, and empathetic with a hospice heart Valid RN license in the state of employment At least 2 years of healthcare experience, preferably in hospice or home health Respected within the community
