We have recently engaged with a very progressive, well run and financially stable Hospice organization in the Walton County area of Georgia. They are a nurse-owned and operated organization that is committed to patient satisfaction, clinical excellence, and quality care for each member of the family.

Our client is offering the following benefits:

• Annual Salary: $80,000-$90,000 +
• Health Insurance-Employer Pays % COBRA
• Dental And Vision Insurance – Employer Pays % COBRA
• Mileage Reimbursement-$0.48 Per Mile
• PTO-14 Days
• Paid Holidays- 7 Federal
• 401K with 3% Match

Responsibilities:

• The RN Case Manager Will Maintain a Case Load of About 12-14 Patients
• Must Have MatrixCare Experience
• Patients Per Day-4 on Average
• The RN Case Manager Will be Responsible for Back-up Call

Requirements:

• Registered Nurse in the State of Georgia
• 2-3 year of Hospice or Palliative Care Experience-Preferred
• 2 years of in-Home Patient Care Experience-Required
• Familiarity with EMR System MatrixCare-Preferred
