Hospice Case Manager RN

One of Georgia's top health systems is looking for a dedicated Hospice Case Manager RN for an 8-hour morning to afternoon shift This role involves delivering patient care across various settings, including homes, hospitals, and long-term care facilities within the hospice service area. The Hospice Case Manager RN will collaborate with a multidisciplinary hospice team to ensure effective patient care and desired outcomes, considering patients' age, gender, and cultural diversity.

Responsibilities include supervising Hospice aides, providing on-call coverage, and contributing to quality nursing care through performance improvement processes. Candidates must have a Georgia RN licensure, current BLS certification, and an Associate's degree. A minimum of one year of clinical nursing experience is required, with a preference for those with hospice experience and CHPN certification.
