What You Will Do:

As a hospice case manager for Amedisys, you will have a special opportunity to provide, coordinate and direct the provision of hospice nursing and palliative care to terminally ill patients in their home. You will closely collaborate with the attending physician, patients and their families, and other members of the patient care team to provide and maintain continuity of patient care to achieve excellent pain and symptom management and high quality end-of-life care for the patient.

  • Assess and reassess all physical, environmental and emotional factors to determine hospice needs for patients in the home setting.

  • Works in cooperation with the family/caregiver and IDT to meet the emotional needs of the patient/family/caregiver.

  • Utilize a combination of agency resources and nationally recognized standards of practice to achieve excellent pain and symptom management and high quality end-of-life care.

  • Coordinate the total Plan of Care and maintains continuity of patient care by liaising with appropriate staff and the physician.

  • Actively participates in Interdisciplinary Team (IDT) conferences.

  • Provide supervision and evaluation of aides and nurses.

  • Attend funerals/calling hours when appropriate as assigned by the bereavement coordinator.


  • Current and unencumbered license to practice as a Registered Nurse specific to that state the employee is assigned to work by the Company.

  • Minimum of one (1) year of experience as a registered nurse

  • Current CPR certification

  • Valid Driver's License and reliable transportation and agency required liability insurance to complete home visits


  • Bachelor of Science in Nursing (BSN) degree

  • Previous home health or hospice experience

  • Certified Hospice and Palliative Nurse (CHPN)

Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) RN-Care Manager in Leominster. Community Healthlink's Behavioral Health Community Partner (BHCP) program is a new, innovative Care Management approach for MassHealth members with complex medical, behavioral health and social needs who are part of the new Accountable Care Organization (ACO) and or a Managed Care Organization (MCO). Multidisciplinary BHCP teams will play a key role in providing high quality integrated care; functioning as a bridge between clients, behavioral health providers and medical providers. Team members will use their expertise to support individuals to navigate health and social systems in order to help members address social determinants health, and improve overall quality of life and health. The BHCP RN Care Manager is a core member of the collaborative care team which includes the enrollee's medical providers and behavioral health providers, and is responsible for supporting and coordinating the mental and physical health care of enrollees assigned. The RN Care Manager works collaboratively with the interdisciplinary team to ensure engaged enrollee's needs are met and care delivery is coordinated across the continuum. Benefits: Compensation based on education and related work experience, licensure and other qualifications Health and Dental insurance (active upon starting) Generous paid sick, personal, holiday, and vacation time CEU benefits Life and Disability insurance 401k with agency contribution Flexible Spending Accounts available Wellness activity discounts Work related travel reimbursement Responsibilities * The RN Care Manager serves as a liaison between the hospital, primary care practices and community agencies or facilities for the exchange of clinical and referral information. * Perform a Comprehensive Assessment and participate in Care Planning. * Conduct medication reconciliation, verify medication orders are appropriate, check for discrepancies, ensure that enrollees understand medication administration and adhere to medication regimen. * Provide Health education and Wellness coaching to help promote healthy activities, manage chronic medical conditions and reduce high risk behaviors. * Engage enrollees in setting wellness goals in Person Centered Care Plan and promote healthy activities such as smoking cessation and weight loss. * Knowledge of and ability to work effectively with community based resources, referral networks, and public/private systems of care. Qualifications * Grade 9: Community RN: Current valid MA RN license * Must have a minimum of two year's experience * Current valid US-issued drivers license and ability to provide a registered, inspected and insured automobile for work related purposes including travel between work sites * Must be able to pass a CORI background check Community Healthlink is an Affirmative Action/ Equal Opportunity Employer.
