We truly hope to find associates to join us with the same compassion and commitment. We are currently seeking a qualified Home Health Aide with a experience with Hospice patients. T The ideal candidate will be able to handle stressful situations with grace and compassion.
This position would provide and/or assist patients/clients with personal care and activities of daily living in their place of residence. Performs duties as assigned/directed by the supervising Registered Nurse (RN). Adapts key functions and duties of the position to address the population served which is a high percentage of adult and geriatric . Must be able to provide own transportation to assignments. Duties may include weekends and on-call assignments on a rotating basis.
ResponsibilitiesThe right candidate is capable of delivering care/service appropriate to patients/customers. Communicates changes in patient status to supervisor. Creates and submits legible, accurate and timely documentation.
Shows sound judgment. Adapts care to individual patient/client needs including age-specific and/or cultural needs. Supports patient rehabilitation, encourages client independence, involves patient in care plan and respects patient's rights.
Performs duties in compliance with regulations, policies and procedures. Demonstrates attention to details for patient's safety and safety of their home environment. Reports unsafe conditions in the home.
Practices effective infection control and adheres to infection control policies and procedures. Demonstrates safe driving record. Practices safe lifting and transferring of patients.
Demonstrates effective body mechanics during provision of care. Complies with policies and procedures related to safety, security and disaster preparedness. Adapts to change in patient assignments.
Makes recommendations for improving patient/client care and/or services. Reports schedule changes. Regularly attends and participates in educational programs.
Identifies areas needed for improving knowledge. Represents the organization as a reflection of its mission. Communicates clearly and positively with patients, families, team members and others.
Displays caring behavior. Keeps patients and/or family informed regarding visit time and schedule changes. QualificationsMust be a Certified Nursing Assistant , or current registered Home Health Aide.
HHA registry preferred. Ability to work independently. Possesses a caring and kind manner toward people.
Demonstrates a positive attitude. Ability to work in and support a team environment. Must be flexible. Must have a valid drivers license.
