Job SummaryThe Hospice Aide (HA) is a member of the interdisciplinary team responsible for assisting patients/families, hired caregivers and facility staff in the provision and teaching of patient personal care needs. The HA provides care under the direction of the Primary Nurse.Certifications, Licenses, Registrations and Requirements * Active HHA Certificate- Required * Active CNA Certificate * Pass OIG/GSA review and all required background checks- Required* CPR/BLS Certification- Required * Current CA Driver's License- Required * Must have provided services for compensation within the last 24 consecutive months * Must be available to work one weekend day (Saturday or Sunday)Essential ResponsibilitiesPersonal Care/Homemaking Services * Provides safe, sensitive and supportive personal care as ordered by the physician and in accordance with the HA plan of care developed by the RN, safe HA practice and the policies and procedures of HSCC. Is attentive to the needs expressed by the patient and family * Develops care activities in coordination with facility staff & hired caregivers as part of the care team and works together in a cooperative and respectful way * Develops and maintains good relationships with facility staff. * Insures that care is provided in a manner that provides satisfaction to the patient and family * Respects the patient's right to make his/her own decisions. * Uses safe body mechanics while performing care * Plans care to maximize patient energy conservation * Demonstrates active listening skills while providing care * Observes and reports patient changes and safety concerns to the Primary Nurse according to standards and procedures * Teaches and supports patients and families regarding patient care, safety, and body mechanics * Demonstrates understanding of reporting and charting protocols in SNFs & RCFEs. * Performs other related job duties as assigned by SupervisorCompliance*
  • Ensures all HIPAA Privacy and Security Regulations, Medicare Conditions of Participation, and all other regulatory/compliance requirements are understood and followedWork Group Communication & Cooperation * Meets regularly with HA Manager for non clinical direction and support. * Works cooperatively with HA Manager and team members. * Submits completed electronic documentation and schedule within the time frames per HSCC policy. Checks voice mail every morning. * Accepts assignments and changes of assignment positively. * Notifies HA Manager of cancellations, schedule changes immediately. * Brings questions/concerns regarding scheduling to HA Manager * Demonstrates understanding of reporting structure. * Follows reporting guidelines and voice mail format promptly and concisely. * Brings questions/concern regarding individual patients to PN* Brings questions/concerns regarding policy, practices, changes, etc to HA Manager. * Visits are planned grouping patients by location and considering facility schedules and patient status. * Documents all contacts and interventions provided per HSCC policies and procedures. * Works cooperatively with PN in scheduling and completion of supervised visits.Attends IDG, All Agency and HA meetings regularly.RequirementsExperience and Skills * Good verbal and written communication skills * Comfort level with using a laptop computer as well as basic computer skills * Bilingual in English/Spanish preferred * Ability to meet the unique care needs of various patients and families * Prefer previous experience using sensitivity and compassion in serving patients and families facing a terminal illness, death and bereavement * Clinical competence in the provision of safe, supportive personal care * Fiscal understanding in the planning and delivery of servicesPhysical Demands * Ability to provide patient care that involves assisting with lifting, repositioning, moving patients and includes bending, reaching and twisting. * Ability to travel to patient locations; includes ability to drive, negotiate stairs and walkways including uneven surfaces at private homes or facilities. * Vision ability to determine patient needs using visual inspection and observation * Hearing ability within normal range to elicit and detect pertinent information while communicating with patients and health team * Speech to verbally communicate to determine and impart information concerning patient status; to complete patient/family teaching and to interact with health care team * Sensory ability to monitor, determine and address patient care needs

    VIEW JOBS 12/29/2019 12:00:00 AM 2020-03-28T00:00 Job Summary The Weekend Visit Nurse (VN) participates in providing staffing coverage including, but not limited to: team visit coverage, RNCM coverage for vacation support/leaves of absence, and Admissions support. The position is 10 hour shifts on Friday, Saturday and Sunday. Working in collaboration with the PCM and Triage, the VN responds to pt/family needs as identified providing phone instruction/support or visits as needed. Care provided is based on an individualized Plan of Care developed by the IDG that reflects pt/family goals. The VN demonstrates: flexibility and ability to manage multiple competing priorities; competence in admissions as well as case management; consistent communication with PCM, Triage and other team members regarding patient care throughout the day, clinical competence in pain and symptom management; and fiscal responsibility in the planning and delivery of services. Essential Responsibilities Patient/family Care * Recognizes the patient and family as the "unit of care" and IDG members * Respects patient right to self-determination and acts as an advocate within the context of the values and ethical standards of HSCC * Makes visits per agency policy * Makes home visit at time of death * Utilizes nursing process in providing primary care to pt/families, and professional patient management to facility staff * Documents all care and care planning per agency policy * Provides nursing interventions for pt/family as ordered by the physician and in accordance with the POC, safe nursing practice and the policies and procedures of HSCC * Monitors management of pain and symptoms in collaboration with patient/family, facility staff, IDG and physician to achieve maximum comfort * Administers medications and treatments as ordered prn * Instructs and supports pt/family unit and facility staff re: symptom mgmt, patient care, disease process & death/dying Admission Visits * Meets with patients & families in all settings to explain services, answer questions and admit patient as appropriate * Develops and implements an individualized, interdisciplinary Plans of Care for patients and families * Uses electronic medical record (EMR) systems effectively and efficiently to complete required documentation in a variety of patient care settings Secondary Responsibilities Quality Improvement * Meets all standards and requirement as indicated by licensing and accrediting agencies * Participates in activities that affirm, enhance and develop the highest quality standards for HSCC * Participates in developing standards for quality patient care within HSCC * Participates in Quality Assessment & Improvement activities as requested * Performs other related job duties as assigned by Supervisor Compliance * Ensures all HIPAA Privacy and Security Regulations, Medicare Conditions of Participation, and all other regulatory/compliance requirements are understood and followed * Submits all appropriate documentation as required for compliance including RN license auto insurance; driver's license, a good driving record, TB checks completed annually Requirements * Required to attend regular meetings and in-services * Critical thinking * Familiarity with electronic charting programs and/or demonstrated keyboard and computer skills sufficient to utilize electronic medical record charting. Education/Experience * Degree in Nursing from an accredited college * 2+ years RN experience in hospice, oncology, medsurg, ICU, ER, skilled nursing facilities or homecare * Acute care experience preferred * Hospice and/or home health experience preferred * End-of-life and pain management experience preferred * Bilingual in English/Spanish preferred but not required. * Previous experience in hospice or palliative care preferred.
