Job Summary:

The Hospice Aide is committed to the philosophy, goals, and objectives of the hospice concept. As a member of the Interdisciplinary Team, the Hospice Aide will provide personal care services to the terminally ill patient and perform related tasks in the home setting in accordance with the plan of treatment as assigned by the RN.

Qualifications:

  • High school diploma or GED.

  • Previous experience as a Hospice Aide or CNA.

  • Completion of a certified training program.

  • Current registration as CNA in the state.

  • Dependable transportation.

  • Working telephone and contact number.

  • Minimal experience working with computers, smart phones, or similar devices.

  • Completion of annual competencies with RN.

Responsibilities:

  • Carries out the duties assigned by the RN in order to increase patient's level of comfort, and to maintain personal hygiene and a safe environment for the patient.

  • Under the direction of a Registered Nurse and in accordance with the written plan of care, the Hospice Aide shall:

  • Give or assist with personal care of the patient and maintain or assist in maintaining cleanliness of patient's immediate surroundings.

  • Assistance with shower/tub bath or total bed bath, as patient desires.

  • Wet shampoo utilizing shampoo board; comb hair.

  • Mouth and tooth care.

  • Skin care and shaving.

  • Nail and food care.

  • Catheter care.

  • Change bed linen.

  • Other personal care duties as assigned.

  • Helps the client to achieve a good quality of life through:

  • Encouraging independence, eating, social contact, etc.

  • Providing companionship.

  • Providing reassurance.

  • Recognizes and reports any changes in the mental or physical status of the patient promptly to the RN, or to the Hospice Aide Lead or DON if RN is unavailable.

  • Maintains awareness of potential anxiety in families.

  • Provides respite relief for primary caregivers as requested.

  • Participates in Interdisciplinary Team meetings.

  • Assists the Registered Nurse with specific assigned procedures.

  • Requests assistance if unable to perform assigned tasks completely.

  • Completes appropriate records in a timely manner.

  • Identifies own educational needs to keep current and upgrade knowledge and skills via in-services and self-study.

Under the supervision of Hospice nurses, the Hospice Aide provides direct patient care and services as established in the patient's Plan of Care. The Hospice Aide must be compassionate, mature and able to handle the needs of a Hospice patient and their family. This employee must also have access to a dependable vehicle and be willing to travel to see patients. Additional Details * Under the direction and ongoing supervision of a registered nurse, provides personal direct patient care services as established and delegated in the Plan of Care on a daily basis. * Reports after patient encounter to the Executive Director, or designee, regarding the patient's condition or environment, following the Care Management process on as needed basis.. * Provides assistance with the following ADLs during each patient visit: mobility, transfers, walking, grooming, bathing, dressing or undressing, eating or toileting. * Performs incidental household services essential to the patient's health care at home conducting household services that are necessary to prevent or postpone institutionalization on as needed basis. * Completes a clinical note for each visit which must be incorporated into the patient's record at least on a weekly basis. * Participates in the in-service program per online educational forum, completing at least six hours of in-services every six months. Must have a minimum of 12 hours of in-service training annually. * Maintains strict patient confidentiality on a regular basis. * Participates in QAPI plan or process identifying any problems and reporting administrator on a daily or as needed basis. * Provides emotional support & compassionate care to the patient & family on regular basis. * Functions as a preceptor to new hires as needed. Qualifications License Requirements * Current CPR certification requirements. * Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation. Additional State Specific Requirements * GA: Required to meet at least one of the following: Georgia certified nursing aide with current certification, or Individuals who have completed and can provide validation or documentation of completion of a home health aide training and competency evaluation program conducted in a Medicare-certified home health agency. The program must be conducted through classroom and supervised practical training totaling at least 75 hours.
