Salary and Benefits

• The pay range for this position is $85,000 - $92,000
• Health Insurance – Employer covers 75%

Responsibilities

• Monday – Friday (1:00 pm – 10:00 pm)
• Providing admissions from the hospital, located at major metro Atlanta hospitals (Northside, Piedmont or St. Josephs)
• If the patient is discharged, then they will be completed at the patients’ home in any of the territories.
• Determine patient eligibility for hospice services.
• Coordinate patient admissions in patient homes.
• Admit patients, inform them of rights and responsibilities, and develop care plans.

Requirements

• Clear and active RN license in the state of Georgia.
• Hospice experience preferred.
• Hospice Admissions experience required.
