Participates in direct patient care providing homemaker/companion services and additional assistance to patients in their homes as outlined by the patient's individual Plan of Care.

Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.


  • Assists patients with homemaking activities which include, but are not limited to, vacuuming, dusting, sweeping or mopping floors, meal preparation, doing dishes, change bed linens, doing laundry and cleaning bathrooms essential to the patient's environmental needs in accordance with the patient's individualized care plan.

  • Assists patients in the community with transportation to Doctor Appointments, errands and grocery shopping essential to the patient's needs in accordance with the patient's individualized care plan.

  • Provides assistance to those who have personal care needs such as, but not limited to, bathing set up, assistant with personal hygiene tasks, and minimal assistance with repositioning and/or walking essential to the patient's physical needs in accordance with the patient's individualized care plan. Hands on care needs will be deferred to licensed staff.

  • Creates and maintain a calm, supportive atmosphere, engaging in patient's personal interest and positive interaction by providing companionship, engaging in social/recreational activities, and assistance with hobbies and/or interest of patient in accordance with the patient's individualized care plan.

  • Observes the patient's physical, mental, emotional, and spiritual conditions and reports any changes to the nurse case manager or Clinical Supervisor/Manager in a timely manner.

  • Documents services provided timely and accurately in accordance with agency policy and personal care plan.

  • Performs clerical support tasks relative to the needs of the agency, such as, scanning medical record, chart audits, data entry, stocking and inventory of supplies/DME, and additional office assistance as needed.

  • Initiates and maintains the Health Technology equipment and electronic health record when Coordinator is not available or assistance is needed.

  • Communicates with staff, physicians, and facilities as needed to obtain require elements for medical record

  • Provides Medical Records information as requested per legal and HIPAA regulations.

  • Abides by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI).

  • Serves a population newborn through geriatric.

  • High School Diploma/GED
  • 6-12 months of relevant experience


  • Previous experience in the healthcare field.


  • Healthcare provider BLS.

  • Kansas State Driver's License

  • Auto Insurance equal to or greater than the Kansas insurance requirements.
