We are a leading health source for our community, offering wellness programs, medical education and illness prevention. We are looking for a Home Health Nurse to join our small and specialized home care health team and provide exceptional care to our patients.

Position Highlights:

  • Monday through Friday - flexible scheduling.

Responsibilities:

Administer skilled nursing care to patients requiring intermittent professional nursing services in accordance with department policies and procedures. Complete initial evaluation visit, regularly re-evaluate the patients nursing needs, initiates the plan of care and furnishes services.

Coordinates services works with physicians and other personnel of changes in patients condition counsels patient and families. Participates in in-service programs, and supervises and teaches other nursing personnel.

Education: Must possess, as a minimum, a Nursing Degree and a current RN license from the state of Colorado.

Experience: Must have a minimum, one (1) year experience in a hospital, skilled nursing facility, or other comparable institution. Wound care experience a plus.

Licenses/Certification: Licensed RN in the State of Colorado. Valid Driver's License, current automobile insurance, reliable transportation, and current CPR certification.

Compensation:

RN - $30.44 - $44.10/hour DOE

Physical Requirements:

Occasionally (0-33%) - Walking, sitting, feeling, crouching, stooping, kneeling, stairs, lifting/lowering (0-50 lbs), carrying (0-50 lbs), pushing/pulling (0-50 lbs)

Frequently (34 - 66%) - Standing, change position, reaching, reach across midline, handling, fingering

Continuously (37 -100%) See with corrective eyewear, hear clearly with assistance

Benefits Eligibility

Medical, dental, vision, health care FSA, dependent care FSA, and Lifestyle Spending Account: All active employees working 40 or more hours per pay period in a Full Time or Part Time position are eligible for benefits on the first of the month after hire.

Full Time staff are automatically enrolled in 401A plan as of date of hire.

Life and AD&D insurance: All active employees working 40 or more hours per pay period are eligible for benefits on the first of the month after hire date.

Short-term and long-term disability: All active employees working 60 or more hours per pay period are eligible for benefits on the first of the month after hire date.
