This position works as a paraprofessional employee who provides comprehensive personal care

and assists with other health care needs for the patient under a written Plan of Care.

JOB DUTIES AND RESPONSIBILITIES:

ESSENTIAL FUNCTIONS:
1.Gives or assists with bathing, dressing, oral hygiene, hair treatments, nails and skin care.
2.Assists with elimination, utilizing a bedpan or urinal, commode or bathroom.
3.Feeds or assist patient with eating and prepares meals including special diets prescribed for the patient.
4.Assists with positioning, transfers and ambulation, with or without assistive devices.
5.Assists patient with prescribed range of motion and prescribed exercise which patient and aide have been taught by designated professional personnel.
6.Remind/assist patient with oral medications that can be self-administered.
7.Assists with application of non-prescription topical ointments.
8.Takes and records temperature, pulse, respiration and blood pressure.
9.Performs routine catheter care, including catheter irrigation by gravity and removal of catheter if instructed to do so by professional staff.
10. Performs clean simple dressing change and decubitus care.
11. Assists patient with long standing ostomy (where function is normal and care is routine). Includes emptying pouch, changing appliance and assisting with preparation of irrigation procedure.
12. Documents tasks performed by telephony/or designated record daily.
13. Applies ace bandage and elastic stockings.
14. Communicates with patient's families and related persons and agency staff to achieve an effective exchange of verbal/nonverbal and written information.
15. Observe and report deviations from the usual physical and mental status of the patient and any unusual occurrence(s) to Supervisor/Case Manager in a timely manner.
16. Provides holistic care to hospice patients.
17. Performs light household homemaking chores in the patient's immediate environment essential for the safety and comfort of the patient and maintenance of a functional, healthful, and clean environment.

QUALIFICATIONS (MINIMUM)

EDUCATION:

High school diploma or GED. Must be able to read, write and speak the English language.

TRAINING AND EXPERIENCE:

Has successfully completed a training and competency evaluation program that meets the standards established by the Department of Health and Human Services, as well as agency performance standards. Certified Nurse Aide (CNA) /Personal Care Aide (PCA) /Home Health Aide (HHA) experience preferred.

Weekday reserve (per-diem) Hospice Liaison is a Registered Nurse position to cover for time off and vacations as well as additional help on busy days. The Hospice Liaison will represent St. Luke's Hospice to build and enhance relationships with referral sources and facilitate the processing of hospice referrals. The Hospice Liaison will use clinical knowledge and empathetic communication skills to explain and educate about hospice services and evaluate patients for hospice eligibility. The Hospice Liaison will collaborate with other VNA departments to assure that the referral sources' needs are met and that patient's progress seamlessly along the continuum of care. He/she will work cooperatively with the Hospice team to identify, plan and pursue growth opportunities. 1. Facilitates processing of hospice referrals and continuity of patient care by acting as the Hospice representative at local health care hospitals, facilities and the community. 2. Communicates with Intake Department and Clinical Managers re: availability of resources and referral capacity. 3. Acts as a resource to hospital staff and the community regarding hospice. 4. Validates referral information. Facilitates development of the initial plan of care with Physicians, including evaluating for hospice eligibility and communicating within the VNA , and with other community resources and vendors to assure continuity and appropriateness of all services to meet the patient's needs. 5. Understands Third party coverage to assure appropriate financial coverage for services ordered. Communicates any financial responsibility to the patient and family as necessary. 6. Develops and in-depth understanding of the needs and preferences of each referral source and the associated opportunities for the VNA to satisfy their needs. 7. Plans and presents educational forums to hospital staff, medical community and community at large as requested. 8. Participates in department initiatives in marketing all VNA programs to the Network and community. 9. Maintain current knowledge of hospice regulations, industry trends and reimbursement issues. PHYSICAL AND SENSORY REQUIREMENTS: Medium Work Exertional activity, Occasionally lift and/or carry 50 lbs. EDUCATION: Graduate of State approved School of Nursing required. Bachelor's degree preferred. TRAINING AND EXPERIENCE: 1. Minimum of 3 years of successful, generalized community health nursing, one year with liaison responsibilities in a similar setting preferred. 2. Must possess initiative, good problem-solving skills, and negotiating abilities. 3. Must have a general familiarity of Microsoft Office and Outlook software. 4. Hospice experience preferred.
