Responsibilities

Administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Patient Services/Clinical Manager. Home Community population served based on the scope of services in the department.
1.Provides care and services in accordance with the physician-ordered plan of care.2. Makes the initial evaluation visit, evaluates patient risk for preventive and rehabilitative nursing procedures, and regularly reevaluates the patient's nursing needs.3. Initiates the plan of care, makes revisions, and updates as necessary.4. Documents patient progression towards clinical goals for each patient, plans care conferences on his/her patients, and conducts discharge planning in a timely manner as per Agency policy.5. Processes orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.6. Coordinates services with interdisciplinary care team and makes appropriate referrals to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills when appropriate.7. Educates and empowers the patient and family/significant others for safe home management and discharge.8. Participates in staff meetings, in-service programs, and adheres to established Agency policies and procedures.9. Conducts patient care conferences on patients assigned to his/her care.10. Participates in onboarding other nursing personnel, peer reviews, and Quality Assessment and Performance Improvement activities as assigned.

Qualifications

  • Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
  • PREFER: Two (2) years' nursing experience
Minimum Knowledge, Skills and Abilities

Requirements:
  • Basic Computer EMR skills

  • Acceptance of philosophy and goals of this Agency.

  • Ability to exercise initiative and independent judgment.

License, Registration, or Certification

Requirements:
  • Valid NYS RN License

  • Valid NYS Driver's License

  • BLS Certification

  • Additional CPR Training as required

  • Proof of car insurance

Per Diem- MON-FRI AND WEEKENDS

Responsibilities The Professional Development Specialist is the clinical expert in evidence-based nursing practice in the areas assigned. The Professional Development Specialist will work under the ANA Scope and Standards of Practice for Nursing Professional Development roles established by the American Nurse's Credentialing Center (ANCC) to include: Educator, Consultant, Researcher, Facilitator, Change Agent, and Leader. The PDS will also exemplify the eight nursing characteristics identified in the AACN Synergy Model (Clinical Judgment, Facilitation of Learning, and Response to Diversity, Clinical Inquiry, Systems Thinking, Collaboration, Advocacy, and Caring Practices). Qualifications Master's degree in Nursing from an accredited program or currently in the process Minimum Experience: 2-5 years of educational experience in an acute care or academic organization or demonstrated expertise in a clinically equivalent role. Required Certification/ Registration: Current New York State Registered Nurses. Certification in Nursing Specialty is required within 6 months of hire.
