Job Summary We are seeking a skilled Registered Nurse to join our team at Apex Home Healthcare. As a Home Health RN, you will provide high-quality nursing care to patients in their homes, working a weekend shift schedule.

Key Responsibilities Initiate, develop, implement, and revise patient care plans in collaboration with physicians and other healthcare professionals. Supervise and provide instruction to home health aides and licensed practical vocational nurses, ensuring compliance with state and federal regulations. Provide accurate and timely documentation of patient visits, including medication reconciliation and OASIS assessments.

Communicate effectively with patients, families, and healthcare team members to ensure quality care and service excellence. Participate in on-call rotation and weekend rotation as needed to meet patient needs. Requirements Current RN licensure in the state of practice.

Current CPR certification. Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
