We are looking for a Home Health Registered Nurse II for Home Health Direct at Home Health Network.

Location: Temecula, CA

Work Schedule: Full time - 80 Biweekly Hours

Shift: Pay Per Visit Employee

This service area incudes the Temecula and the San Bernardino area.

Job Summary:

The Registered Nurse provides infusion services for patients at home based on the orders of a physician or other licensed prescriber. This position assists in the coordination of home health services after patients are referred for service, to assist in the transition from hospital to home care and ensure continuity of care between hospital, clinic setting, SNF, physician's office and home. Responsible for continually re-evaluating the patient's nursing needs.

Essential Functions:

  • Demonstrates knowledge and competency in performing baseline-nursing skills, including assessment, wound care, catheter care and infusion. Maintains clinical competencies according to ministry standards.

  • Initiates and/or administers infusion therapy accurately and timely under the direction of a physician at specific time-points.

  • When indicated coordinates infusion pharmacy services with nursing agencies who will resume care of patient.

  • Collaborates with pharmacists in carrying out the plan of care, in collaboration with the patient, family/caregiver and physician orders, specific care or treatments to be provided, appropriate goals, medications, other disciplines needed and discharge plan.

  • Documents the initiation of infusion services in the Home Health EHR.

  • Implements and provides nursing care, treatment and services according to the plan of care and accepted standards of care to include patient education and any IV or other medications required to be administered by the nurse.

  • Communicates changes in patient status, patient issues and concerns and changes in orders to the Physician and Pharmacist according to ministry guidelines.

  • Practices infection control and universal precautions for self and others in patient care activities.

  • Advocate for quality patient outcomes communicating with authorization nurses and community vendors timely.

  • Maintains patient rights and dignity.

  • Annually completes compliance education and understands applicable policies.

  • Meets the productivity standards of the organization

Skills:

Minimum Position Requirements:

Education: Completes necessary education to attain RN Licensure in state of California

Experience: 1 year Nursing experience in clinical setting

Licenses/Certifications:

  • Current California RN license

  • Current Driver's license and current automobile liability insurance

  • Current basic CPR

Preferred Position Qualifications:

Experience:

  • IV and Central lines experience

  • Intravenous infusion experience

  • Experience with EHR

  • Home health field experience

RNs in this role work autonomously alongside the hospital staff to assess, troubleshoot, and make sound decisions for the critically ill patients. Dialysis Experience is NOT required. *What you can expect:* * *Use your critical care thinking skills. *Whether this is the first or a blip in their chronic treatment, our RNs are experts in educating our patients and families. You'll be able to leverage your nursing skills for patients who are often dealing with multiple co-morbidities and/or acute conditions which require unique treatment plans and the ability to leverage a broad range of nursing skills and knowledge. * *Not just hemodialysis. *Acute RNs may deliver nephrology care across multiple renal therapies: peritoneal, continuous renal replacement and apheresis. * *Autonomy in a hospital setting. *Deliver dialysis to patients at a particular hospital or group of hospitals and become an integral part of their team and ours. As a teammate in an acute setting you work on your own almost all of the time. Each day you assess, troubleshoot, and make sound decisions for critical patients. Our Hospital Services group performs 1.1M treatments annually. * *Dynamic environment.* No two days are ever the same for an acute dialysis nurse. You'll work within a variety of settings (ICU, CCU, ER and more), and you'll be able to keep learning new nursing skills on top of dialysis. * *Schedule:* Provide emergent, lifesaving care during on call shifts. Schedules vary by hospital system. For this role expect to know your weekly schedule in advance, but your daily schedule may change depending on patient needs.
