About This Home Health RN Case Manager Position
HopeHealth is seeking an experienced Registered Nurse (RN) for its affiliate HopeHealth Community VNA to visit patients in Massachusetts.
This position is headquartered in Attleboro, MA.
This is a full-time, 40-hours per week, day shift position that is eligible for benefits.
This position is eligible for a bonus of up to $2,500.
What will you do?
Provide direct patient care utilizing state of the art technology and nursing processes to assess, plan, implement and evaluate plan of care in collaboration with patient's physician.
Connect with your patients to see their outcomes first-hand and the difference you are making in their lives.
Teach and educate your patients and their caregivers effective patient self-care techniques and approaches.
Responsible for the assessment and evaluation of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments and adaptive equipment.
Develop patient specific plan of care and goals.
Follows physician orders and physical therapy treatment plans for patients
Identifies equipment needs of patient, requesting physician orders for equipment, working with equipment companies in ordering equipment for patient, and educating patient and caregiver on new equipment
Regularly re-evaluate patients to identify progress towards goal achievement; reviews OASIS answers, coding, and sequencing of diagnoses to ensure accuracy; obtains orders and updates Plans of Care as needed throughout the episode of care.
Participates effectively as part of an interdisciplinary team to assure that patient services are delivered in a timely manner
Documents patient care and data accurately, thoroughly and by the end of the scheduled workday.
Supervise and train licensed practical nurses and home health aides.
Secures positive clinical and quality outcomes according to state and national benchmarks.
Case management and collaborate with interdisciplinary teams.
Participate in on-call, weekend, and holiday rotations as needed.
Facilitate and participate in weekly case conferences.
What are the qualifications for this job?
Graduate of an accredited nursing school
Licensed as a Registered Nurse in MA
Minimum of 1-year clinical experience required
Excellent assessment, clinical and communication skills
Dually Licensed in Rhode Island and Massachusetts a plus
Experience with wound care, medication management, pulmonary, and cardiac care a plus
Experience in a Homecare, Med-Surg, ICU, acute care, or sub-acute care setting preferred
Experience with case management, prioritization of care, and care coordination preferred
