We are seeking a Physical Therapist to: Join our world-class team of doctors, nurses and other health care professionals in providing everything from preventive health care to highly specialized programs. Provide quality care in a compassionate patient- and family- centered environment. Plan, evaluate and administer physical therapy services and care to patients per physician#s orders in their home on an intermittent basis according to the policies of the Agency. Direct physical therapy assistants in providing treatments to clients following Agency guidelines. Participate in continuous quality improvement activities and educational experiences in support of departmental philosophy and objectives, as well as, Health System initiatives. AGE SPECIFIC CARE: Provides age and culturally appropriate assessment/screening, interpretation of clinical and laboratory data and develops and implements age appropriate interventions and plans of care including education within the parameters of his/her position responsibilities and licensure. EDUCATION/TRAINING: Graduate of an accredited Physical Therapy program. LICENSURE/CERTIFICATION: Current license from the State of Illinois. An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment. #An applicant who fails to proved this documentation will not be hired. A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided. Bi-annual CPR certification required. Valid state driver#s license and proof of liability insurance. EXPERIENCE/SKILLS/ABILITIES: Ability to effectively communicate orally and in writing in a caring, considerate and concerned manner.# Willingness to accept responsibility. Ability to direct ancillary staff and organize work assignments. Leadership skills essential to the position. Knowledge of JCAHO, CMS, IDPH rules and regulations preferred.
